Description
Overview of the medicine
Nalipsy 200 mg Tablet contains Gabapentin, an anticonvulsant and neuropathic pain agent. It is used to treat partial seizures in epilepsy and neuropathic pain, such as postherpetic neuralgia.
Uses & Indications
Dosage
Adults
Epilepsy: Initially 300 mg three times daily; may be titrated up to 1800-3600 mg/day. Postherpetic Neuralgia: Initially 300 mg once daily, titrated to 300 mg three times daily, or higher based on response.
Elderly
Dosage adjustment may be necessary due to age-related decline in renal function. Start with a lower dose.
Renal_impairment
Dosage must be reduced in patients with impaired renal function (creatinine clearance <80 mL/min).
How to Take
Oral. Can be taken with or without food. Swallow whole with water. Do not crush, chew, or break.
Mechanism of Action
Gabapentin's precise mechanism is not fully understood, but it is thought to modulate excitatory neurotransmitter release, possibly by binding to the α2δ (alpha-2-delta) subunit of voltage-gated calcium channels in the central nervous system, reducing calcium influx and subsequent neurotransmitter release.
Pharmacokinetics
Onset
Typically within a few days to weeks for therapeutic effect, depending on titration.
Excretion
Excreted unchanged by the kidneys.
Half life
5-7 hours in adults with normal renal function.
Absorption
Bioavailability is dose-dependent and non-linear, decreasing with increasing dose. Peak plasma concentrations are reached in 2-3 hours.
Metabolism
Not significantly metabolized in humans.
Side Effects
Contraindications
- Hypersensitivity to gabapentin or any component of the formulation.
- Acute pancreatitis (rare, but reported).
Drug Interactions
Antacids
May decrease gabapentin bioavailability. Take gabapentin at least 2 hours after antacids.
Alcohol/CNS Depressants
Increased risk of CNS depression.
Opioids (e.g., Morphine)
May increase gabapentin concentrations, leading to increased CNS depression. Monitor for sedation.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include dizziness, double vision, slurred speech, drowsiness, lethargy, and mild diarrhea. Management is supportive care and hemodialysis in severe cases, as gabapentin is dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Gabapentin is excreted in breast milk; caution advised. Consult a doctor.

Lab Monitoring
- No routine lab monitoring is required for gabapentin in patients with normal renal function. Creatinine clearance should be monitored in patients with renal impairment.
Doctor Notes
- Gradual titration is crucial to minimize adverse effects.
- Monitor renal function, especially in elderly and renally impaired patients.
- Educate patients on potential CNS side effects and the importance of not abruptly discontinuing the drug.
