Description
Overview of the medicine
Napa Rapid Actizorb 500 mg Tablet is a fast-acting formulation of Paracetamol, designed for quick relief from pain and fever. The Actizorb technology facilitates rapid absorption, leading to a quicker onset of action compared to standard paracetamol tablets.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg (1-2 tablets) every 4 to 6 hours as needed. Maximum 4000 mg (8 tablets) in 24 hours.
Elderly
No specific dose adjustment is required, but caution is advised in patients with impaired renal or hepatic function.
Renal_impairment
Dose interval should be extended in severe renal impairment (e.g., 500 mg every 6-8 hours for CrCl 10-50 mL/min, and every 8 hours for CrCl <10 mL/min).
How to Take
Take orally with a glass of water, with or without food. Do not exceed the recommended dose or frequency.
Mechanism of Action
Paracetamol (Acetaminophen) primarily acts by inhibiting prostaglandin synthesis, mainly in the central nervous system (CNS). It exhibits analgesic and antipyretic effects by reducing the production of prostaglandins that sensitize pain receptors and mediate fever in the hypothalamus. The 'Actizorb' technology enhances tablet disintegration and dissolution, leading to faster absorption and a quicker onset of action.
Pharmacokinetics
Onset
Within 15-30 minutes for analgesic and antipyretic effects due to rapid absorption.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma elimination half-life is typically 1 to 4 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, especially with Actizorb formulation. Peak plasma concentrations are achieved faster (typically within 15-30 minutes) than conventional paracetamol.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small amount is metabolized by cytochrome P450 enzymes to a toxic intermediate, N-acetyl-p-benzoquinone imine (NAPQI), which is normally detoxified by conjugation with glutathione.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any component of the formulation.
- Severe hepatic impairment or active liver disease.
Drug Interactions
Alcohol
Increased risk of hepatotoxicity with chronic alcohol consumption.
Cholestyramine
Reduces paracetamol absorption if taken within one hour.
Warfarin and other coumarins
May enhance the anticoagulant effect, increasing the risk of bleeding with prolonged regular use.
Metoclopramide or Domperidone
May increase the rate of paracetamol absorption.
Certain antiepileptics (e.g., phenobarbital, phenytoin, carbamazepine)
May increase the risk of paracetamol-induced liver toxicity due to increased formation of toxic metabolites.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose can cause severe, potentially fatal, liver damage. Symptoms may include nausea, vomiting, abdominal pain, and loss of appetite. Immediate medical attention is crucial. N-acetylcysteine (NAC) is the antidote, most effective when administered within 8 hours of ingestion.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation at recommended therapeutic doses. It crosses the placental barrier and is excreted in breast milk in small amounts. However, consult a doctor before use.

Lab Monitoring
- Liver function tests (especially in prolonged use, high doses, or suspected overdose)
- Renal function tests (in patients with pre-existing renal impairment)
Doctor Notes
- Emphasize strict adherence to dosing limits to prevent hepatotoxicity.
- Counsel patients to check other OTC products for paracetamol content to avoid accidental overdose.
- Advise caution in patients with pre-existing liver or kidney disease, or chronic alcohol users.
