Overview of the medicine
Naproxen is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, fever, and inflammation. It works by blocking the body's production of certain natural substances that cause inflammation. It is commonly used for conditions like arthritis, menstrual cramps, and various types of pain.
Uses & Indications
Dosage
Adults
For pain/dysmenorrhea: 250 mg or 500 mg initially, then 250 mg every 6-8 hours as needed, not to exceed 1250 mg/day initially, then 1000 mg/day. For chronic conditions: 250 mg or 500 mg twice daily.
Elderly
Lower doses may be required due to increased risk of side effects, particularly renal impairment. Initial dose 250 mg twice daily.
Renal_impairment
Use with caution, reduced dosage may be necessary. Not recommended for severe renal impairment (CrCl < 30 mL/min).
How to Take
Take with food or milk to reduce gastrointestinal upset. Swallow the tablet whole with a full glass of water. Do not crush, chew, or break.
Mechanism of Action
Naproxen works by reversibly inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins from arachidonic acid. Prostaglandins are mediators of inflammation, pain, and fever. By inhibiting their synthesis, naproxen reduces these symptoms.

Onset
Pain relief within 1 hour.
Excretion
Primarily excreted in the urine (about 95%) as naproxen, 6-O-desmethylnaproxen, and their conjugates. A small amount is excreted in feces.
Half life
Approximately 12-17 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached in 2-4 hours.
Metabolism
Extensively metabolized in the liver by demethylation and subsequent conjugation.
Side Effects
Contraindications
- •Hypersensitivity to naproxen or other NSAIDs
- •Asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •Perioperative pain in the setting of coronary artery bypass graft (CABG) surgery
- •Active gastrointestinal bleeding or ulcer
- •Severe renal or hepatic impairment
- •Third trimester of pregnancy
Drug Interactions
Lithium
Increased plasma lithium concentrations.
Methotrexate
Increased methotrexate toxicity.
Other NSAIDs/Aspirin
Increased risk of gastrointestinal adverse effects.
Warfarin/Anticoagulants
Increased risk of bleeding.
Diuretics/ACE Inhibitors
Reduced hypotensive and diuretic effects; increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, heartburn, indigestion, nausea, vomiting, abdominal pain. Severe cases may involve gastrointestinal bleeding, acute renal failure, hypertension, respiratory depression, and coma. Management involves symptomatic and supportive care. Gastric lavage and activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Avoid use in the third trimester of pregnancy due to risk of premature closure of the fetal ductus arteriosus. Use during the first and second trimesters only if clearly needed and the potential benefit outweighs the potential risk to the fetus. Naproxen is excreted in breast milk; use with caution in nursing mothers, or consider an alternative.
