Description
Overview of the medicine
Napier 500 mg tablet contains Paracetamol, an analgesic and antipyretic used for symptomatic relief of mild to moderate pain and fever.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg every 4-6 hours as needed. Maximum 4000 mg (4g) in 24 hours.
Elderly
Same as adult dose, but caution is advised in frail elderly patients with hepatic or renal impairment.
Renal_impairment
Dosage adjustment may be necessary. For CrCl 10-50 mL/min, administer every 6 hours; for CrCl <10 mL/min, administer every 8 hours. Max 3g/day.
How to Take
Oral. Can be taken with or without food. If stomach upset occurs, take with food or milk.
Mechanism of Action
Paracetamol acts primarily in the central nervous system, inhibiting prostaglandin synthesis by blocking cyclooxygenase (COX) enzymes, particularly COX-2. It also has a weak effect on peripheral COX.
Pharmacokinetics
Onset
Within 30-60 minutes for analgesia, 15-30 minutes for antipyresis.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1 to 3 hours in adults.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily metabolized in the liver by conjugation with glucuronide and sulfate. A small amount is metabolized via the cytochrome P450 pathway to a toxic intermediate, NAPQI, which is normally detoxified by glutathione.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any component of the formulation
- Severe hepatic impairment or active liver disease
Drug Interactions
Alcohol
Increased risk of liver toxicity, especially with chronic heavy alcohol consumption.
Warfarin
May increase the anticoagulant effect of warfarin, increasing the risk of bleeding, especially with prolonged use or high doses.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, abdominal pain, anorexia, malaise, and sweating, potentially leading to hepatic necrosis. Treatment involves gastric lavage, activated charcoal, and administration of N-acetylcysteine (NAC).

Generally considered safe for use during pregnancy and lactation at recommended doses, but consult a doctor.
