Description
Overview of the medicine
Naproxen is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, inflammation, and fever.
Uses & Indications
Dosage
Adults
For chronic conditions: 250-500 mg twice daily, or 500 mg once daily. Maximum 1000 mg/day for chronic use. For acute pain/dysmenorrhea: 500 mg initially, then 250 mg every 6-8 hours as needed. Maximum 1250 mg on the first day, then 1000 mg/day.
Elderly
Lower doses are generally recommended due to increased risk of adverse effects; monitor renal function.
Renal_impairment
Reduced dosage or avoid use. Not recommended for patients with severe renal impairment (creatinine clearance <30 mL/min).
How to Take
Take orally, preferably with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water.
Mechanism of Action
Inhibits cyclooxygenase enzymes (COX-1 and COX-2) to reduce prostaglandin synthesis, thereby reducing inflammation, pain, and fever.
Pharmacokinetics
Onset
Onset of analgesia typically occurs within 1-2 hours.
Excretion
Primarily excreted in the urine (approximately 95%) as metabolites and unchanged drug.
Half life
The plasma elimination half-life is approximately 12-17 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma levels are reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver by demethylation and subsequent conjugation with glucuronic acid.
Side Effects
Contraindications
- Known hypersensitivity to naproxen, aspirin, or other NSAIDs
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Active gastrointestinal bleeding or peptic ulcer disease
- Severe renal impairment
- Severe hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
Drug Interactions
SSRIs
Increased risk of gastrointestinal bleeding.
Lithium
Increased plasma lithium levels, potentially leading to toxicity.
Methotrexate
Increased methotrexate toxicity.
Diuretics and ACE inhibitors
Reduced antihypertensive and diuretic effects, increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, heartburn, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management involves gastric lavage, activated charcoal, and symptomatic and supportive care. Hemodialysis is not effective.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to the risk of premature closure of the fetal ductus arteriosus. Use with caution in the first two trimesters and during lactation after consulting a doctor, weighing the potential benefits against the risks.
