Description
Overview of the medicine
Napren (Naproxen) is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and fever. It works by blocking the body's production of certain natural substances that cause inflammation.
Uses & Indications
Dosage
Adults
For mild to moderate pain, initial dose 500 mg, followed by 250 mg every 6-8 hours as needed. For chronic conditions like arthritis, 250-500 mg twice daily. Maximum daily dose 1250 mg.
Elderly
Lower doses may be required due to increased risk of side effects. Start with the lowest effective dose.
Renal_impairment
Use with caution. Dosage reduction may be necessary in patients with mild to moderate renal impairment. Contraindicated in severe renal failure.
How to Take
Take orally with food or milk to minimize gastrointestinal upset. Swallow whole with a glass of water.
Mechanism of Action
Naproxen inhibits cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins are mediators of inflammation, pain, and fever. By inhibiting their synthesis, naproxen reduces these symptoms.
Pharmacokinetics
Onset
Pain relief typically within 1 hour, anti-inflammatory effects may take longer.
Excretion
Primarily excreted in the urine (about 95%) as naproxen and its metabolites.
Half life
Approximately 12-17 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract.
Metabolism
Extensively metabolized in the liver by demethylation and subsequent conjugation.
Side Effects
Contraindications
- Known hypersensitivity to naproxen or other NSAIDs (e.g., aspirin-induced asthma)
- Active peptic ulcer or gastrointestinal bleeding
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- Coronary artery bypass graft (CABG) surgery (peri-operative pain)
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of GI ulceration and bleeding.
Diuretics (e.g., furosemide)
Reduced diuretic effect, increased risk of renal impairment.
Anticoagulants (e.g., warfarin)
Increased risk of bleeding.
Selective Serotonin Reuptake Inhibitors (SSRIs)
Increased risk of GI bleeding.
ACE Inhibitors / Angiotensin Receptor Blockers (ARBs)
Reduced antihypertensive effect, increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, heartburn, nausea, vomiting, epigastric pain, and rarely gastrointestinal bleeding. Management involves symptomatic and supportive treatment, including gastric lavage or activated charcoal.
Pregnancy & Lactation
Pregnancy Category C (first and second trimesters), Category D (third trimester). Should be avoided in the third trimester due to potential fetal harm. Small amounts are excreted in breast milk; use with caution or consider alternatives during lactation.
