Description
Overview of the medicine
Naproxen is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of mild to moderate pain, fever, and inflammation. It works by blocking the body's production of certain natural substances that cause inflammation.
Uses & Indications
Dosage
Adults
For arthritis: 250 mg to 500 mg twice daily. For pain/dysmenorrhea: 500 mg initially, then 250 mg every 6-8 hours as needed. Max daily dose: 1250 mg.
Elderly
Lower doses may be required due to increased risk of adverse effects; dose adjustment based on renal function.
Renal_impairment
Use with caution and at reduced doses (e.g., 250 mg twice daily initially) in patients with mild to moderate renal impairment. Contraindicated in severe renal impairment.
How to Take
Take orally with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a full glass of water. Do not crush, chew, or break extended-release tablets.
Mechanism of Action
Naproxen inhibits prostaglandin synthesis by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes. This leads to reduced formation of prostaglandin precursors, thereby reducing pain, inflammation, and fever.
Pharmacokinetics
Onset
Analgesic effect begins within 1 hour, anti-inflammatory effect takes longer (up to 2 weeks for full effect in chronic conditions).
Excretion
Primarily excreted in the urine (approximately 95% as naproxen or its conjugates); a small amount excreted in feces.
Half life
Approximately 12-17 hours, allowing for twice-daily dosing.
Absorption
Rapid and complete absorption from the gastrointestinal tract; peak plasma concentrations reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver by demethylation, then conjugated with glucuronic acid.
Side Effects
Contraindications
- •Known hypersensitivity to naproxen, aspirin, or other NSAIDs
- •History of asthma, urticaria, or allergic-type reactions after aspirin or other NSAIDs
- •Peri-operative pain in the setting of coronary artery bypass graft (CABG) surgery
- •Active or recurrent peptic ulcer/hemorrhage
- •Severe heart failure
- •Severe renal impairment (creatinine clearance <30 mL/min)
- •Severe hepatic impairment
- •Third trimester of pregnancy
Drug Interactions
Lithium
Increased plasma lithium levels, potentially leading to toxicity.
Methotrexate
Increased plasma methotrexate levels, potentially leading to toxicity.
Beta-blockers
Reduced antihypertensive effect.
Diuretics (e.g., Furosemide)
Reduced diuretic and antihypertensive effects.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Other NSAIDs or corticosteroids
Increased risk of gastrointestinal adverse effects.
Selective Serotonin Reuptake Inhibitors (SSRIs)
Increased risk of gastrointestinal bleeding.
Antiplatelet agents (e.g., Aspirin, Clopidogrel)
Increased risk of gastrointestinal bleeding.
ACE inhibitors and Angiotensin II Receptor Blockers (ARBs)
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, epigastric pain, lethargy, and coma. Management involves symptomatic and supportive care. Gastric lavage and activated charcoal may be considered within 1 hour of ingestion.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to risk of premature closure of fetal ductus arteriosus. Use with caution during the first and second trimesters and during lactation only if the potential benefit justifies the potential risk to the fetus/infant. Consult a doctor.
