Overview of the medicine
Naproxzia Plus 500 mg Tablet is a combination medicine containing Naproxen, a non-steroidal anti-inflammatory drug (NSAID), and Esomeprazole, a proton pump inhibitor (PPI). It is used to relieve pain, inflammation, and stiffness associated with various conditions while protecting the stomach from the gastrointestinal side effects of Naproxen.
Uses & Indications
Dosage
Adults
One tablet (Naproxen 500 mg/Esomeprazole 20 mg) twice daily, at least 30 minutes before meals.
Elderly
Use with caution; consider lower doses, especially in patients with impaired renal or hepatic function.
Renal_impairment
Use with caution and monitor renal function. Not recommended for severe renal impairment (CrCl <30 mL/min).
How to Take
Take the tablet whole with water, at least 30 minutes before a meal. Do not crush, chew, or split the tablet.
Mechanism of Action
Naproxen inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of pain and inflammation. Esomeprazole, a proton pump inhibitor, decreases gastric acid secretion by irreversibly binding to and inhibiting the H+/K+-ATPase pump in gastric parietal cells, thereby reducing the risk of NSAID-induced gastric ulcers.
Pharmacokinetics
Onset
Pain relief from Naproxen typically begins within 1 hour. Gastric acid suppression from Esomeprazole begins within 1 hour.
Excretion
Naproxen and its metabolites are primarily excreted in the urine (approximately 95%). Esomeprazole and its metabolites are excreted primarily via the urine (approx. 80%) and feces (approx. 20%).
Half life
Naproxen: Approximately 12-17 hours. Esomeprazole: Approximately 1-1.5 hours.
Absorption
Naproxen is well absorbed from the GI tract, with peak plasma concentrations occurring within 2-4 hours. Esomeprazole is rapidly absorbed, with peak plasma concentrations within 1-2 hours.
Metabolism
Naproxen is primarily metabolized in the liver by demethylation and subsequent conjugation. Esomeprazole is extensively metabolized in the liver via the cytochrome P450 system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Known hypersensitivity to Naproxen, Esomeprazole, or any component of the formulation
- •Patients with a history of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
- •In the setting of coronary artery bypass graft (CABG) surgery
- •Third trimester of pregnancy
Drug Interactions
Warfarin
Increased risk of bleeding due to Naproxen's antiplatelet effect and displacement from plasma protein binding.
Clopidogrel
Increased risk of bleeding with Naproxen.
SSRIs/SNRIs
Increased risk of GI bleeding with Naproxen.
Methotrexate
Increased methotrexate toxicity due to reduced renal clearance by Naproxen.
Atazanavir/Nelfinavir
Coadministration with Esomeprazole is not recommended.
Diuretics (e.g., Furosemide)
Reduced diuretic and antihypertensive effects due to Naproxen.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of Naproxen overdose include drowsiness, heartburn, nausea, vomiting, epigastric pain, GI bleeding. Symptoms of Esomeprazole overdose are generally mild. Treatment is primarily supportive and symptomatic; gastric lavage and activated charcoal may be considered. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category C in early pregnancy, D in third trimester. Avoid use in late pregnancy due to risk of premature closure of the fetal ductus arteriosus. Use with caution during lactation; Naproxen is excreted in breast milk, and Esomeprazole also likely passes into breast milk.
