Description
Overview of the medicine
Naptodin Plus is a combination medicine containing Naproxen, a non-steroidal anti-inflammatory drug (NSAID), and Esomeprazole, a proton pump inhibitor (PPI). It is used to relieve pain and inflammation while protecting the stomach lining from potential side effects of Naproxen.
Uses & Indications
Dosage
Adults
One tablet once daily, preferably before a meal, for the shortest duration possible.
Elderly
Use lowest effective dose for shortest duration. Close monitoring for adverse effects.
Renal_impairment
Use with caution. Dosage adjustment may be required in severe impairment.
How to Take
Take the tablet whole with water. Do not crush, chew, or split the tablet. Administer at least 30 minutes before a meal.
Mechanism of Action
Naproxen inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of inflammation, pain, and fever. Esomeprazole selectively inhibits the H+/K+-ATPase proton pump in gastric parietal cells, blocking the final step of acid secretion, leading to reduced gastric acidity.
Pharmacokinetics
Onset
Pain relief from Naproxen within 1-2 hours. Acid suppression from Esomeprazole within 1 hour.
Excretion
Mainly excreted in urine, some in feces.
Half life
Naproxen: approximately 12-17 hours. Esomeprazole: approximately 1.5 hours.
Absorption
Naproxen is rapidly and completely absorbed from the GI tract. Esomeprazole is also rapidly absorbed, but bioavailability increases with repeated dosing.
Metabolism
Both are primarily metabolized in the liver by CYP enzymes, Naproxen mainly by CYP2C9, Esomeprazole mainly by CYP2C19 and CYP3A4.

Contraindications
- Hypersensitivity to Naproxen, Esomeprazole, or other NSAIDs
- Active gastrointestinal bleeding or ulcer
- Severe renal or hepatic impairment
- Third trimester of pregnancy
- Severe heart failure
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs

Increased digoxin levels.
Lithium
Increased lithium levels.
Tacrolimus
Increased risk of nephrotoxicity.
Clopidogrel
Increased risk of bleeding.
Methotrexate
Increased methotrexate toxicity.

Increased risk of bleeding.
Diuretics/ACE inhibitors
Reduced efficacy and increased risk of renal impairment.
SSRIs (e.g., Citalopram, Fluoxetine)

Overdose
Symptoms of overdose may include drowsiness, lethargy, nausea, vomiting, epigastric pain, GI bleeding. Management includes symptomatic and supportive care. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Not recommended during the third trimester of pregnancy due to risk of premature closure of the fetal ductus arteriosus. Use with caution in other trimesters if benefits outweigh risks. Naproxen is excreted in breast milk; Esomeprazole may also be present. Consult a doctor.
