Description
Overview of the medicine
Nasoflu 50 mcg Nasal Spray contains fluticasone propionate, a corticosteroid, used to relieve nasal symptoms such as sneezing, runny nose, itching, and congestion caused by allergic rhinitis.
Uses & Indications
Dosage
Adults
Initially, 2 sprays (50 mcg/spray) into each nostril once daily. May be reduced to 1 spray per nostril once daily for maintenance, or increased to 2 sprays twice daily if symptoms are severe.
Elderly
Similar to adult dosage. No specific dose adjustment is generally required.
Renal_impairment
No dose adjustment is required for patients with renal impairment.
How to Take
For intranasal use only. Shake well before use. Prime the pump by spraying several times into the air until a fine mist appears if the spray has not been used for several days. Tilt head slightly forward, insert nozzle into nostril and spray while gently breathing in through the nose. Repeat for the other nostril. Avoid spraying into the eyes.
Mechanism of Action
Fluticasone propionate is a potent glucocorticoid that exhibits anti-inflammatory effects. It reduces inflammation in the nasal passages by inhibiting the release of inflammatory mediators from various cell types involved in the allergic response.
Pharmacokinetics
Onset
Clinical effects typically observed within 12-24 hours, with maximal relief taking several days.
Excretion
Mainly excreted in feces as inactive metabolites, with minimal renal excretion.
Half life
Plasma half-life of approximately 7-16 hours.
Absorption
Low systemic absorption (bioavailability less than 2%) following intranasal administration.
Metabolism
Primarily hepatic metabolism via the cytochrome P450 CYP3A4 enzyme.
Side Effects
Contraindications
- Hypersensitivity to fluticasone propionate or any component of the formulation.
- Untreated localized infection involving the nasal mucosa.
Drug Interactions
Ritonavir
Coadministration with ritonavir can significantly increase fluticasone propionate plasma concentrations, leading to systemic corticosteroid effects. Concomitant use is not recommended.
Ketoconazole
Concomitant use with other strong CYP3A4 inhibitors (e.g., ketoconazole) may increase systemic exposure to fluticasone propionate, though less significantly than ritonavir.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose is unlikely due to the low systemic absorption. However, chronic excessive dosing may lead to signs of hypercorticism. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy and lactation only if the potential benefit justifies the potential risk to the fetus or infant. Consult a healthcare professional.
