Overview of the medicine
Navigo 40 mg Injection contains Esomeprazole, a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions caused by excess stomach acid, such as gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
The usual recommended dose is 40 mg once daily by intravenous injection or infusion.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Administer slowly over at least 3 minutes for intravenous injection or over 10 to 30 minutes for intravenous infusion. The reconstituted solution should be used within 12 hours.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the parietal cells of the stomach. This inhibition blocks the final step in gastric acid secretion, leading to a profound and prolonged reduction of gastric acid.
Pharmacokinetics
Onset
Within 30 minutes for acid suppression.
Excretion
Excreted predominantly by the kidneys (approximately 80%) and in feces (approximately 20%) as metabolites.
Half life
Approximately 1.0 to 1.5 hours in healthy individuals.
Absorption
Rapidly and almost completely absorbed after intravenous administration, reaching peak plasma concentrations within minutes.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Tacrolimus
Increased serum concentrations of tacrolimus.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased plasma levels of methotrexate, particularly at high doses.
Atazanavir/Nelfinavir
Reduced plasma concentrations of these antiretrovirals, potentially leading to loss of therapeutic effect.
Storage
Store below 30°C, protected from light and moisture. Do not freeze. Reconstituted solution should be used within 12 hours.
Overdose
Limited experience with overdose. Symptoms may include gastrointestinal disturbances and weakness. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Esomeprazole is likely excreted in human milk; caution is advised when administered to a nursing woman.
