Overview of the medicine
NCR-FAST 20 mg Tablet contains Tenoxicam, a non-steroidal anti-inflammatory drug (NSAID) used for symptomatic relief in various inflammatory and degenerative rheumatic diseases and painful conditions.
Uses & Indications
Dosage
Adults
Usually 20 mg once daily. For acute conditions, 40 mg once daily for the first 1-2 days, then 20 mg daily.
Elderly
Careful dosing recommended, usually 20 mg once daily, considering potential renal impairment.
Renal_impairment
Reduce dose in severe renal impairment (creatinine clearance <25 mL/min); maximum 10 mg daily.
How to Take
Take orally, preferably after food, to minimize gastrointestinal upset. Swallow the tablet whole with water.
Mechanism of Action
Tenoxicam inhibits prostaglandin synthesis by non-selectively blocking cyclooxygenase (COX-1 and COX-2) enzymes. Prostaglandins are mediators of inflammation, pain, and fever.

Onset
Within 30 minutes to 2 hours for analgesic effect.
Excretion
Mainly through urine (two-thirds) and bile (one-third) as inactive metabolites.
Half life
Approximately 60-75 hours (very long).
Absorption

Extensively metabolized in the liver, primarily by hydroxylation.
Side Effects
Contraindications
- Active peptic ulcer or gastrointestinal bleeding
- History of gastrointestinal bleeding or perforation related to previous NSAID therapy
- Severe heart failure
- Severe renal impairment
- Severe hepatic impairment
- Third trimester of pregnancy
- Known hypersensitivity to Tenoxicam or other NSAIDs (including aspirin-induced asthma)
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased plasma methotrexate levels and toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
Reduced efficacy of diuretics/ACE inhibitors and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Antiplatelet agents (e.g., Aspirin, Clopidogrel)
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, epigastric pain, lethargy, drowsiness. Management involves symptomatic and supportive care. Gastric lavage or activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Not recommended during the third trimester of pregnancy due to risk of premature closure of ductus arteriosus and renal dysfunction in the fetus. Should be avoided during lactation as NSAIDs can pass into breast milk.
