Overview of the medicine
Nelod 10 mg Tablet contains Felodipine, a calcium channel blocker, used primarily to treat high blood pressure (hypertension) and to prevent chest pain (angina pectoris). It works by relaxing blood vessels, making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
Hypertension: Initial dose 5 mg once daily, adjusted according to response, usually at 2-week intervals. Usual maintenance dose is 5-10 mg once daily, max 20 mg once daily. Angina: Initial dose 5 mg once daily, maintenance dose typically 10 mg once daily.
Elderly
A lower initial dose (e.g., 2.5 mg or 5 mg once daily) should be considered, with careful titration.
Renal_impairment
No dose adjustment is generally required in patients with renal impairment.
How to Take
Take Nelod 10 mg Tablet orally once daily, preferably in the morning. It can be taken with or without food. Swallow the tablet whole; do not crush, chew, or divide. Avoid taking with grapefruit juice.
Mechanism of Action
Felodipine is a dihydropyridine calcium channel blocker. It selectively binds to L-type calcium channels in the vascular smooth muscle, inhibiting the influx of extracellular calcium ions. This results in arterial vasodilation, leading to reduced peripheral vascular resistance and a decrease in blood pressure. It also increases coronary blood flow, relieving angina.

Excretion
Approximately 70% excreted in urine and 10% in feces, primarily as inactive metabolites.
Half life
Terminal elimination half-life is 10-16 hours.
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract, but undergoes extensive first-pass metabolism. Absolute bioavailability is approximately 15%.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4. All identified metabolites are inactive.

Drug Interactions
Beta-blockers and other Antihypertensives

Storage

Overdose
Symptoms of overdose include severe hypotension (low blood pressure) and bradycardia (slow heart rate). Management involves supportive measures, including intravenous fluids to increase blood volume and vasopressors if hypotension persists. Careful monitoring of vital signs is essential.
Pregnancy & Lactation
Pregnancy Category C. Nelod (Felodipine) is generally not recommended during pregnancy due to potential risks to the fetus. It is also not recommended during breastfeeding as felodipine is excreted in breast milk.
