Description
Overview of the medicine
Neobetrin 5 mg Ointment is a triple antibiotic formulation containing Neomycin Sulfate, Bacitracin Zinc, and Polymyxin B Sulfate. It is used to prevent and treat minor skin infections caused by bacteria, such as cuts, scrapes, and burns.
Uses & Indications
Dosage
Adults
Apply a small amount to the affected area 1 to 3 times daily, after cleaning the area.
Elderly
Same as adult dosage.
Renal_impairment
No specific dosage adjustment required for topical use due to minimal systemic absorption.
How to Take
For external use only. Clean the affected area thoroughly before applying a thin layer of ointment. May be covered with a sterile bandage.
Mechanism of Action
Neomycin binds to the 30S ribosomal subunit, inhibiting bacterial protein synthesis. Bacitracin inhibits bacterial cell wall synthesis. Polymyxin B disrupts the integrity of the bacterial outer membrane by binding to phospholipids, leading to cell leakage and death.
Pharmacokinetics
Onset
Within hours of application for antimicrobial action.
Excretion
Primarily through local action; minimal systemic excretion.
Half life
Not clinically significant due to minimal systemic absorption.
Absorption
Minimal systemic absorption through intact skin. Absorption may increase with compromised skin barrier (e.g., burns, severe dermatitis).
Metabolism
Not extensively metabolized systemically due to minimal absorption.
Side Effects
Contraindications
- Hypersensitivity to Neomycin, Bacitracin, Polymyxin B, or any component of the formulation.
- Deep or puncture wounds, animal bites, or serious burns.
Drug Interactions
Other topical agents
Avoid concomitant use with other topical medications on the same area unless directed by a doctor, as it may alter absorption or efficacy.
Nephrotoxic or Ototoxic Drugs (systemic)
Though systemic absorption is minimal, prolonged or extensive use, especially on compromised skin, could theoretically increase systemic absorption of neomycin, which is known for nephrotoxicity and ototoxicity. Caution with other nephrotoxic/ototoxic drugs.
Storage
Store below 30°C. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause systemic effects due to minimal absorption. Excessive application may lead to skin irritation. If ingested, seek medical attention.
Pregnancy & Lactation
Use during pregnancy and lactation only if clearly needed and advised by a doctor. Minimal systemic absorption reduces risk, but caution is advised.
