Overview of the medicine
Neofenac-SR 100 mg Capsule contains Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and stiffness caused by various conditions such as arthritis, acute musculoskeletal pain, and dysmenorrhea. The sustained-release formulation allows for once-daily dosing.
Uses & Indications
Dosage
Adults
The usual adult dose is 100 mg once daily. It should be taken whole with food or milk.
Elderly
No specific dose adjustment is usually required, but caution should be exercised due to increased risk of adverse effects, especially renal impairment. Consider the lowest effective dose.
Renal_impairment
Use with caution. In severe renal impairment (CrCl <30 mL/min), it is generally contraindicated. For moderate impairment, dose reduction may be necessary.
How to Take
Take the capsule whole with a full glass of water, preferably with food or milk, to minimize gastrointestinal discomfort. Do not crush, chew, or break the capsule.
Mechanism of Action
Diclofenac inhibits cyclooxygenase (COX-1 and COX-2) enzymes, leading to a reduced synthesis of prostaglandins. Prostaglandins are key mediators of inflammation, pain, and fever. By reducing their production, diclofenac exerts its anti-inflammatory, analgesic, and antipyretic effects.
Pharmacokinetics
Onset
Onset of analgesic action typically occurs within 30-60 minutes, with sustained relief due to the SR formulation.
Excretion
Approximately 60% of the dose is excreted in the urine as metabolites, and about 35% is eliminated in the bile/feces.
Half life
The elimination half-life of diclofenac is approximately 1-2 hours, but its therapeutic effect is prolonged with the SR formulation due to continuous drug release.
Absorption
Well absorbed orally. Peak plasma concentrations are achieved within 4-5 hours for sustained-release formulations, providing prolonged systemic exposure.
Metabolism
Extensively metabolized in the liver, primarily via hydroxylation and subsequent glucuronidation. CYP2C9 is the main enzyme involved.
Side Effects
Contraindications
- •Hypersensitivity to diclofenac, aspirin, or other NSAIDs
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •Active gastrointestinal bleeding or peptic ulceration
- •Severe hepatic or renal impairment
- •Severe heart failure (NYHA Class II-IV)
- •Third trimester of pregnancy
- •Coronary artery bypass graft (CABG) surgery (peri-operative pain)
Drug Interactions
SSRIs
Increased risk of gastrointestinal bleeding.
Lithium
Diclofenac may increase plasma lithium levels, leading to toxicity.
Methotrexate
Increased methotrexate toxicity due to reduced renal clearance.
Corticosteroids
Increased risk of gastrointestinal ulceration and bleeding.
Diuretics and ACE inhibitors
Reduced diuretic and antihypertensive effects; increased risk of renal impairment.
Warfarin and other anticoagulants
Increased risk of bleeding.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Rare cases of acute renal failure, hypertension, respiratory depression, and coma have been reported. Management is symptomatic and supportive. Gastric lavage or activated charcoal may be considered within 1 hour of ingestion.
Pregnancy & Lactation
Avoid use in the third trimester of pregnancy due to potential for premature closure of the fetal ductus arteriosus. Use in other trimesters or during lactation only if the potential benefit outweighs the potential risk, and under strict medical supervision. NSAIDs may impair female fertility.
