Overview of the medicine
Neoflox (Ciprofloxacin) is a fluoroquinolone antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
250-750 mg orally twice daily, depending on the type and severity of infection. For uncomplicated UTI: 250 mg twice daily for 3 days. For more severe infections: 500-750 mg twice daily for 7-14 days.
Elderly
Similar to adults, but dosage adjustment may be needed based on renal function and overall health status.
Renal_impairment
Dosage reduction is generally required for creatinine clearance < 50 mL/min. For CrCl 30-60 mL/min: 250-500 mg every 12 hours. For CrCl <30 mL/min: 250-500 mg every 18-24 hours.
How to Take
Take orally with or without food. Avoid taking with dairy products (milk, yogurt) or calcium-fortified juices alone. Take 2 hours before or 6 hours after antacids containing magnesium or aluminum, or iron/zinc supplements to ensure proper absorption.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Varies by infection type and severity, but symptom improvement can be seen within hours to days.
Excretion
Primarily renal (50-70% as unchanged drug) and to a lesser extent fecal (15%).
Half life
Approximately 3-5 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract; bioavailability is approximately 70-80%.
Metabolism
Partially metabolized in the liver (15-30%) to less active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to ciprofloxacin or other fluoroquinolones
- •Concomitant use with Tizanidine
- •History of tendinitis or tendon rupture associated with quinolone use
- •Children under 18 years (unless benefits outweigh risks in specific severe infections)
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing the risk of bleeding. Monitor INR/PT.
Tizanidine
Increased risk of hypotension and sedation. Concomitant use is contraindicated.
Theophylline
Increased serum levels of theophylline, leading to potential toxicity (nausea, vomiting, seizures). Monitor theophylline levels.
Corticosteroids
Increased risk of tendinitis and tendon rupture when co-administered with corticosteroids.
Antacids/Iron/Zinc supplements
Reduced oral absorption of ciprofloxacin. Administer ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach and sight of children.
Overdose
In case of overdose, gastric lavage and symptomatic supportive treatment should be initiated. Monitor ECG and renal function. Hemodialysis or peritoneal dialysis is not effective in removing ciprofloxacin from the body.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if potential benefits justify the potential risk to the fetus. Ciprofloxacin is excreted into breast milk; caution is advised during lactation, or consider an alternative medicine or temporary discontinuation of breastfeeding.
