Description
Overview of the medicine
Metronidazole is an antibiotic and antiprotozoal medication used to treat various infections including anaerobic bacterial infections, amoebiasis, giardiasis, and trichomoniasis. The 200 mg suspension is particularly suitable for pediatric use.
Uses & Indications
Dosage
Adults
For amoebiasis: 400-800 mg three times daily for 5-10 days. For anaerobic infections: 400 mg three times daily. Pediatric: Amoebiasis: 35-50 mg/kg/day in 3 divided doses for 5-10 days. Giardiasis: 15 mg/kg/day in 3 divided doses for 5-7 days.
Elderly
Lower doses may be considered in patients with hepatic impairment; otherwise, adult dosage.
Renal_impairment
No dosage adjustment generally needed for renal impairment, but caution with severe impairment.
How to Take
Oral, taken with or after food to minimize gastrointestinal upset. Shake suspension well before use.
Mechanism of Action
Metronidazole is a nitroimidazole that works by being reduced inside anaerobic cells and protozoa. This reduction generates reactive nitro radicals that damage DNA, inhibit nucleic acid synthesis, and ultimately lead to cell death.

Varies by infection type, generally within hours to days for clinical effect.
Excretion
Mainly renal (60-80% as metabolites and unchanged drug); some fecal excretion.
Half life
Approximately 8 hours (range 6-12 hours).
Absorption
Readily and almost completely absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Primarily hepatic, undergoing hydroxylation and oxidation to produce active metabolites (e.g., 2-hydroxymethylmetronidazole).
Side Effects
Contraindications
- Hypersensitivity to metronidazole or other nitroimidazole derivatives.
- First trimester of pregnancy (relative contraindication, generally avoided).
- Active central nervous system disease.
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia) when consumed with alcohol. Avoid alcohol during and for at least 48-72 hours after metronidazole therapy.
Lithium
May increase serum lithium levels, leading to lithium toxicity.
Warfarin
Metronidazole can potentiate the anticoagulant effect of warfarin and other oral anticoagulants, increasing the risk of bleeding. Close INR monitoring is required.
Storage
Store below 30°C in a dry place, away from direct sunlight. Do not freeze. Keep out of reach of children.

Symptoms include nausea, vomiting, ataxia, disorientation, and seizures. Treatment is symptomatic and supportive; there is no specific antidote.
