Description
Overview of the medicine
Neoprox 40 mg Suspension contains Esomeprazole, a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It is prescribed for conditions like gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. Zollinger-Ellison syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No specific dosage adjustment required for elderly patients with normal renal function.
Renal_impairment
No dosage adjustment required for patients with renal impairment.
How to Take
For oral suspension, mix the powder with the recommended amount of water as per package instructions and take at least one hour before a meal. Do not chew or crush the granules if any.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically and irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This inhibition prevents the final step of acid production, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Approximately 80% is excreted as metabolites in the urine and the remainder in the feces.
Half life
Approximately 1.3 hours
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% after a single 40 mg dose, increasing to 89% after repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole or any other benzimidazole substituted proton pump inhibitors.
- Coadministration with Nelfinavir.
Drug Interactions
Digoxin
Increased digoxin absorption due to increased gastric pH.
Warfarin
Increased INR/prothrombin time, requiring monitoring.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition.
Methotrexate
Possible increased methotrexate levels, especially with high doses.
Storage
Store unreconstituted powder below 30°C, protected from light and moisture. Reconstituted suspension should be stored at room temperature (15-30°C) and used within 28 days. Do not freeze.
Overdose
Limited experience with overdose. Symptoms may include drowsiness, tachycardia, headache, and confusion. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Category C in pregnancy. Use only if potential benefit outweighs the potential risk to the fetus. Excreted in breast milk; caution advised during lactation.
