Description
Overview of the medicine
Nepafix TS is an ophthalmic solution that combines an aminoglycoside antibiotic (Tobramycin) and a corticosteroid (Dexamethasone) to treat bacterial eye infections and associated inflammation. It is effective against a broad range of susceptible ocular bacterial pathogens.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the conjunctival sac of the affected eye(s) every 4 to 6 hours. For severe disease, instill 1-2 drops every 2 hours for the first 24 to 48 hours, then reduce frequency as the infection subsides.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment due to minimal systemic absorption.
How to Take
Wash hands thoroughly before use. Tilt your head back, pull down the lower eyelid to create a small pouch, and instill the prescribed number of drops. Avoid touching the dropper tip to the eye or any other surface to prevent contamination. Close your eye gently for a few minutes.
Mechanism of Action
Tobramycin inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, leading to bacterial cell death. Dexamethasone is a potent synthetic corticosteroid that suppresses inflammatory responses by inhibiting the release of inflammatory mediators and reducing capillary permeability.
Pharmacokinetics
Onset
Rapid relief of symptoms usually begins within hours to a few days, depending on the severity of the condition.
Excretion
Primarily local elimination from the ocular surface. Any systemically absorbed portion is rapidly excreted, mainly via kidneys.
Half life
Systemic half-life is not clinically relevant for topical ocular administration due to minimal absorption.
Absorption
Minimal systemic absorption occurs after topical ocular administration. Most of the active ingredients remain localized in the ocular tissues.
Metabolism
Minimal systemic metabolism. Dexamethasone undergoes hepatic metabolism, but systemic levels are negligible with ocular drops.
Side Effects
Contraindications
- Epithelial herpes simplex keratitis (dendritic keratitis).
- Vaccinia, varicella, and many other viral diseases of the cornea and conjunctiva.
- Mycobacterial infection of the eye.
- Fungal diseases of ocular structures.
- Hypersensitivity to any component of the medication.
Drug Interactions
Other topical ophthalmic medications
If more than one topical ophthalmic product is being used, they should be administered at least 5 minutes apart.
Other topical ophthalmic corticosteroids
Concurrent use with other steroid eye drops may increase the risk of elevated intraocular pressure.
Storage
Store at room temperature (below 30°C). Protect from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Accidental overdose with topical ocular administration is unlikely to cause systemic toxicity due to limited absorption. If swallowed, gastric lavage may be considered in cases of large ingestion, though it's an unlikely scenario.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Dexamethasone may pass into breast milk; use with caution in lactating mothers. Consult a doctor before use.
