Description
Overview of the medicine
Nepafenac is a non-steroidal anti-inflammatory drug (NSAID) used to treat pain and inflammation associated with cataract surgery.
Uses & Indications
Dosage
Adults
One drop in the affected eye(s) three times daily starting 1 day prior to cataract surgery, continued on the day of surgery, and for up to 2 weeks post-surgery.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
No specific dose adjustment is required as systemic exposure is negligible.
How to Take
For ophthalmic use only. Shake well before use. Do not touch the dropper tip to any surface to avoid contamination. If more than one topical ophthalmic drug is being used, they should be administered at least 5 minutes apart.
Mechanism of Action
Nepafenac is a prodrug that rapidly penetrates the cornea and is converted by hydrolases into amfenac, a potent cyclooxygenase (COX) inhibitor. Amfenac then inhibits the activity of prostaglandin H synthase (cyclooxygenase), an enzyme essential for prostaglandin production. Prostaglandins are mediators of inflammation.
Pharmacokinetics
Onset
Significant reduction in pain and inflammation typically seen within hours to days of treatment initiation.
Excretion
Primarily excreted renally as metabolites, with negligible systemic exposure.
Half life
Amfenac (active metabolite) has a plasma half-life of approximately 13 minutes, but its ocular presence is longer.
Absorption
Rapidly absorbed into the cornea after topical ocular administration. It is a prodrug that is converted to amfenac.
Metabolism
Nepafenac is metabolized in ocular tissues by hydrolases to form amfenac.
Side Effects
Contraindications
- Hypersensitivity to the active substance or to any of the excipients.
- Patients with a history of hypersensitivity reactions to other NSAIDs (e.g., aspirin, ibuprofen).
Drug Interactions
Topical Corticosteroids
Concomitant use with topical corticosteroids may increase the risk of corneal adverse events in susceptible patients.
Other Topical Ophthalmic NSAIDs
Avoid concomitant use with other topical NSAIDs.

Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Ophthalmic overdose is unlikely to cause systemic toxicity due to limited systemic absorption. If an overdose occurs, flush the eye(s) with lukewarm water.

Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution is advised during lactation as it is unknown if Nepafenac is excreted in human milk.
