Description
Overview of the medicine
Neuropen is a reusable injection pen device used for the self-administration of Somatropin, a recombinant human growth hormone. It is prescribed for growth hormone deficiency in children and adults, and other conditions requiring growth hormone therapy.
Uses & Indications
Dosage
Adults
Dosage individualized based on patient weight and clinical response, typically 0.2-0.6 mg/day subcutaneously.
Elderly
Lower initial doses may be considered, carefully monitored for side effects.
Renal_impairment
No specific dose adjustments needed for mild to moderate impairment, but careful monitoring is advised.
How to Take
Administer subcutaneously. Patients or caregivers should be trained by a healthcare professional on proper injection technique using the Neuropen device. Rotate injection sites.
Mechanism of Action
Somatropin is a polypeptide hormone of recombinant DNA origin that stimulates skeletal and somatic growth. It binds to specific growth hormone receptors on target cells, activating intracellular signaling pathways that lead to cell proliferation and protein synthesis.
Pharmacokinetics
Onset
Growth-promoting effects are gradual and sustained over time.
Excretion
Excreted primarily through the kidneys.
Half life
Approximately 2-3 hours for unbound Somatropin; longer when bound to GH-binding protein.
Absorption
Bioavailability typically around 80% after subcutaneous administration.
Metabolism
Primarily metabolized in the liver and kidneys by proteolytic enzymes.
Side Effects
Contraindications
- •Active malignancy (any evidence of tumor growth)
- •Acute critical illness (e.g., following open heart surgery, accidental trauma)
- •Diabetic retinopathy (proliferative or severe non-proliferative)
- •Closed epiphyses (in pediatric patients for growth promotion)
Drug Interactions
Insulin
Somatropin may decrease insulin sensitivity. Diabetic patients may require increased insulin dosage.
Oral estrogens
May decrease serum IGF-1 levels. Higher Somatropin doses may be needed in women on oral estrogens.
Glucocorticoids
May inhibit the growth-promoting effects of Somatropin. Dosage adjustment of Somatropin may be required.
Thyroid hormone
Hypothyroidism should be corrected before Somatropin therapy. Concomitant use may require thyroid hormone dosage adjustment.
Storage
Store Somatropin cartridges refrigerated (2-8°C). Do not freeze. Protect from light. After first use/reconstitution, follow specific instructions for remaining shelf life at room temperature or refrigeration.
Overdose
Acute overdose may lead to hypoglycemia initially, followed by hyperglycemia. Long-term overdose may result in signs and symptoms of gigantism/acromegaly. Treatment is supportive.
Pregnancy & Lactation
Category C. Use only if potential benefit outweighs the risk to the fetus. Caution advised during lactation; it is unknown if Somatropin is excreted in human milk.
