Description
Overview of the medicine
Tacrolimus 0.03% ophthalmic suspension is an immunosuppressant eye drop used to reduce inflammation in severe allergic conjunctivitis or dry eye disease (in some cases).
Uses & Indications
Dosage
Adults
1 drop in each affected eye twice daily.
Elderly
Same as adults.
Renal_impairment
Generally no dose adjustment required for ophthalmic use due to minimal systemic absorption.
How to Take
For ophthalmic use only. Shake well before use. Apply to the conjunctival sac. Avoid touching the dropper tip to the eye or any other surface.
Mechanism of Action
It inhibits calcineurin, thereby preventing T-lymphocyte activation and reducing inflammation.
Pharmacokinetics
Onset
Days to weeks for full effect.
Excretion
Mainly fecal, if systemically absorbed, but systemic absorption from ophthalmic use is negligible.
Half life
Systemic half-life is long, but ocular pharmacokinetics are localized.
Absorption
Minimal systemic absorption after ophthalmic administration.
Metabolism
Primarily hepatic (CYP3A4) if systemically absorbed, but systemic absorption from ophthalmic use is negligible.
Side Effects
Contraindications
- Hypersensitivity to tacrolimus or any component of the suspension
- Active ocular infections
Drug Interactions
Other Eye Drops
Wait at least 5 minutes before applying other eye drops.
Systemic CYP3A4 inhibitors
Although systemic absorption is minimal, avoid concomitant use with systemic CYP3A4 inhibitors (e.g., ketoconazole, itraconazole) if possible, or monitor closely.
Storage
Store below 25°C, protected from light and in a dry place. Do not refrigerate.
Overdose
Overdose is unlikely with ophthalmic use. In case of excessive use, rinse the eye with water.
Pregnancy & Lactation
Use with caution; consult a doctor before use during pregnancy or lactation. Should only be used if clearly necessary after assessing risks and benefits.
