Overview of the medicine
Newcip 500 mg Tablet contains Ciprofloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections including urinary tract infections, respiratory tract infections, skin and soft tissue infections, bone and joint infections, and gastrointestinal infections.
Uses & Indications
Dosage
Adults
Typically 250-750 mg twice daily, depending on the infection type and severity. Duration of treatment varies.
Elderly
No specific dosage adjustment is required based on age, but renal function should be considered.
Renal_impairment
Dosage adjustment is necessary for patients with severe renal impairment (creatinine clearance < 30 mL/min). A reduced dose or extended dosing interval may be required.
How to Take
Swallow the tablet whole with a glass of water. It can be taken with or without food, but avoid taking it with dairy products (milk, yogurt) or calcium-fortified juices as these can reduce absorption. Take at least 2 hours before or 6 hours after antacids containing magnesium, aluminum, calcium, or iron supplements.
Mechanism of Action
Ciprofloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Within 0.5-2 hours of administration.
Excretion
Primarily excreted unchanged by the kidneys (50-70%) and to a lesser extent via the feces.
Half life
Approximately 3-5 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract; absolute bioavailability is approximately 70-80%. Peak plasma concentrations are reached 0.5-2 hours after oral administration.
Metabolism
Undergoes limited hepatic metabolism (10-20%) to several less active metabolites.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- Concomitant administration with tizanidine.
- History of tendinitis or tendon rupture associated with quinolone use.
- Children and adolescents (under 18 years), except for specific severe infections where benefits outweigh risks.
Drug Interactions
NSAIDs
Potential for increased CNS stimulation and convulsions.
Warfarin
Enhanced anticoagulant effects, requiring INR monitoring.
Tizanidine
Concomitant use is contraindicated due to increased plasma concentrations of tizanidine.
Cyclosporine
Increased serum creatinine and potential for nephrotoxicity.
Theophylline
Increased plasma levels of theophylline, potentially leading to toxicity.
Antacids, Iron/Zinc Supplements, Sucralfate, Didanosine
Reduced ciprofloxacin absorption. Administer ciprofloxacin at least 2 hours before or 6 hours after these agents.

Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.

In case of overdose, gastric lavage and supportive care are recommended. Hydration should be maintained. Hemodialysis or peritoneal dialysis is not effective in removing ciprofloxacin.
