Description
Overview of the medicine
Nexol (Esomeprazole) is a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions like heartburn, gastroesophageal reflux disease (GERD), and stomach ulcers.
Uses & Indications
Dosage
Adults
Typically 20 mg or 40 mg once daily, depending on the condition. For GERD, 20-40 mg once daily for 4-8 weeks. For H. pylori eradication, 20 mg twice daily for 7-14 days with antibiotics.
Elderly
No specific dose adjustment is generally required, but caution should be exercised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take orally, at least one hour before a meal. Swallow capsules whole. Do not chew or crush. For patients who have difficulty swallowing, capsules may be opened and the contents mixed with soft food.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion. This leads to a sustained decrease in gastric acid production.
Pharmacokinetics
Onset
Acid suppression effect begins within 1 hour.
Excretion
Primarily excreted in urine (about 80%) as metabolites, and to a lesser extent in feces (about 20%).
Half life
Approximately 1.0-1.5 hours (plasma half-life), but the duration of acid suppression is much longer.
Absorption
Rapidly absorbed after oral administration, peak plasma concentration reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased digoxin levels.
Warfarin
Increased INR and prothrombin time.
Tacrolimus
Increased tacrolimus levels.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased levels of methotrexate.
Nelfinavir/Atazanavir
Reduced antiviral effect due to decreased absorption.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms are generally mild and may include drowsiness, tachycardia, nausea, diarrhea, and headache. Management is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy: Category C. Use only if potential benefit justifies the potential risk to the fetus. Lactation: Excreted in breast milk. Use with caution during breastfeeding.
