Description
Overview of the medicine
Amlodipine is a dihydropyridine calcium channel blocker used to treat hypertension (high blood pressure) and various forms of angina (chest pain). It works by relaxing blood vessels, which lowers blood pressure and increases the supply of blood and oxygen to the heart.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, maximum 10 mg once daily. For Nexovas-10, it's typically 10 mg once daily as per physician's advice.
Elderly
Initial dose 2.5 mg once daily, adjust with caution based on individual response.
Renal_impairment
No dosage adjustment generally required.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This reduces peripheral vascular resistance, leading to a decrease in blood pressure. In angina, it increases myocardial oxygen delivery by dilating coronary arteries and arterioles.

Onset
1-2 hours for antihypertensive effect.
Excretion
Primarily excreted via urine (60% as metabolites, 10% as unchanged drug).
Half life
30-50 hours.
Absorption
Well absorbed orally, peak plasma concentrations reached 6-12 hours after oral dosing. Bioavailability is 64-90%.
Metabolism
Extensively metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to amlodipine, other dihydropyridines, or any component of the formulation.
- •Severe hypotension.
- •Cardiogenic shock.
- •Obstruction of the outflow tract of the left ventricle (e.g., high grade aortic stenosis).
- •Haemodynamically unstable heart failure after acute myocardial infarction.
Drug Interactions
Tacrolimus
Amlodipine may increase tacrolimus blood levels; monitor tacrolimus levels and adjust dose as needed.
Simvastatin
Amlodipine may increase simvastatin exposure; limit simvastatin dose to 20 mg/day.
Clarithromycin
Increased risk of hypotension and acute kidney injury.
CYP3A4 Inducers (e.g., Rifampicin)
May decrease amlodipine plasma concentrations.
CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
May increase amlodipine exposure, potentially requiring dose reduction.
Storage
Store below 30°C, in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptom: Excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management: Clinically significant hypotension requires active cardiovascular support including monitoring of cardiac and respiratory function, elevation of extremities, and management of circulating fluid volume and urine output. A vasopressor may be helpful. Intravenous calcium gluconate may reverse the effects of calcium entry blockade. Dialysis is not beneficial.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk to fetus. Small amounts excreted in breast milk; caution advised during lactation.
