Description
Overview of the medicine
Nicodex 0.1/0.3 eye drop is a combination medicine containing a corticosteroid (Dexamethasone) and an aminoglycoside antibiotic (Tobramycin). It is used to treat inflammatory ocular conditions where a bacterial infection or a risk of bacterial infection exists.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the conjunctival sac of the affected eye(s) every 4 to 6 hours. For severe cases, dosage may be increased to 1-2 drops every 2 hours for the first 24 to 48 hours, then decreased as inflammation subsides.
Elderly
No specific dose adjustment required for elderly patients, use adult dosage.
Renal_impairment
No specific dose adjustment required as systemic absorption is minimal.
How to Take
For ophthalmic use only. Shake well before use. Do not touch the dropper tip to any surface to avoid contamination. Apply pressure to the tear duct for a minute or two after instilling drops to minimize systemic absorption.
Mechanism of Action
Dexamethasone is a potent corticosteroid that suppresses the inflammatory response through various mechanisms, including inhibition of prostaglandin and leukotriene synthesis. Tobramycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, leading to bacterial cell death.
Pharmacokinetics
Onset
Within minutes for anti-inflammatory effects; antibacterial effects develop over hours.
Excretion
Systemically absorbed portions excreted primarily via kidneys. Local drug removed via tear film and ocular surface turnover.
Half life
Not well-defined for ocular topical application due to minimal systemic absorption. Local ocular tissue half-life varies.
Absorption
Minimal systemic absorption after ocular administration. Local absorption into ocular tissues.
Metabolism
Systemically absorbed dexamethasone is metabolized in the liver; tobramycin is not extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to any component of the medication.
- •Epithelial herpes simplex keratitis (dendritic keratitis).
- •Vaccinia, varicella, and many other viral diseases of the cornea and conjunctiva.
- •Mycobacterial infection of the eye.
- •Fungal diseases of ocular structures.
- •Untreated purulent infection of the eye.
Drug Interactions
Other ophthalmic preparations
If using other eye drops, wait at least 5 minutes between applications. Using multiple topical corticosteroids may increase ocular pressure.
Storage
Store at room temperature (15°C-30°C). Protect from light and moisture. Do not refrigerate or freeze.
Overdose
Accidental overdose is unlikely with ophthalmic use due to limited absorption. Symptoms might include increased localized adverse effects like eye irritation, itching, or redness. Rinse eye with lukewarm water.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Dexamethasone and tobramycin are excreted in human milk, but systemic absorption from ophthalmic use is low. Consult a doctor.
