Description
Overview of the medicine
Nilcid 480 mg Suspension is an antacid and anti-flatulent medication used to relieve symptoms of heartburn, indigestion, and excess gas. It works by neutralizing stomach acid and reducing gas bubbles, providing fast and effective relief.
Uses & Indications
Dosage
Adults
10-20 mL (2-4 teaspoonfuls) orally, four times a day, preferably between meals and at bedtime, or as directed by a physician.
Elderly
Same as adult dosage, or as directed by a physician, with caution in those with renal impairment.
Renal_impairment
Use with caution in severe renal impairment due to potential for magnesium accumulation. Dosage adjustment may be required; consult a physician.
How to Take
Shake the bottle well before each use. Take orally, preferably between meals and at bedtime for optimal relief. Do not take within 1-2 hours of other medications unless advised by a doctor.
Mechanism of Action
Aluminum hydroxide and magnesium hydroxide act as antacids by neutralizing hydrochloric acid in the stomach, increasing gastric pH. Simethicone is an anti-foaming agent that reduces the surface tension of gas bubbles in the gastrointestinal tract, allowing them to combine into larger bubbles that are easier to pass or absorb.
Pharmacokinetics
Onset
Within 15 minutes of administration.
Excretion
Unabsorbed antacids are excreted in feces. Absorbed magnesium is rapidly excreted by the kidneys.
Half life
Not directly applicable for antacids due to minimal systemic absorption; elimination half-life for absorbed magnesium is 2-6 hours.
Absorption
Aluminum and magnesium are minimally absorbed (approx. 15-30% of magnesium) from the GI tract. Simethicone is not absorbed systemically.
Metabolism
Absorbed magnesium is not metabolized. Aluminum is primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to aluminum hydroxide, magnesium hydroxide, simethicone, or any excipients.
- Severe renal impairment (due to risk of hypermagnesemia).
- Hypophosphatemia.
Drug Interactions
Digoxin
May decrease absorption of digoxin. Separate administration.
Iron supplements
Reduced absorption of iron. Separate administration by at least 2 hours.
Tetracyclines/Fluoroquinolones
Antacids can chelate with these antibiotics, reducing their absorption. Administer at least 2 hours before or 4-6 hours after antacid.
Thyroid hormones (Levothyroxine)
Reduced absorption. Separate administration by several hours.
Storage
Store in a cool and dry place, below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms of overdose may include severe diarrhea, constipation, nausea, vomiting, and electrolyte imbalances (e.g., hypermagnesemia, hypophosphatemia). Management is symptomatic and supportive, including hydration and correction of electrolyte disturbances. In severe cases, dialysis may be required for hypermagnesemia.
Pregnancy & Lactation
Generally considered safe for short-term use during pregnancy and lactation, but use under medical supervision. Magnesium may cause diarrhea in breastfed infants, and aluminum accumulation is a theoretical concern with prolonged use.

Clinical Trials
The efficacy and safety of combination antacids (aluminum hydroxide, magnesium hydroxide) with simethicone are well-established through numerous clinical studies for the symptomatic relief of acid-related dyspepsia and flatulence.
Lab Monitoring
- Not routinely required for short-term use.
- For prolonged use or in patients with renal impairment, monitor serum magnesium and phosphate levels.
Doctor Notes
- Counsel patients on lifestyle modifications (diet, eating habits, sleep position) to manage acid reflux.
- Advise patients to separate antacid administration from other medications by at least 1-2 hours to avoid potential drug interactions.
- Caution in patients with chronic renal impairment due to the magnesium content and potential for accumulation.
Patient Guidelines
- Shake the bottle well before each use to ensure uniform suspension.
- Do not exceed the recommended dose without consulting a healthcare professional.
- If symptoms persist for more than 2 weeks or worsen, consult a doctor.
- Keep out of reach of children.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Nilcid 480 mg Suspension is not known to affect the ability to drive or operate machinery. However, if you experience any side effects like dizziness, avoid such activities.
Lifestyle Advice
- Avoid trigger foods such as spicy, fatty, acidic foods, caffeine, and alcohol.
- Eat smaller, more frequent meals instead of large ones.
- Avoid eating late at night and elevate the head of the bed during sleep to reduce reflux.
