Overview of the medicine
Nimesulide is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation.
Uses & Indications
Dosage
Adults
100 mg twice daily. Maximum daily dose is 200 mg. Duration of treatment should be as short as possible and not exceed 15 days.
Elderly
Consider lower doses or extended dosing intervals, especially in elderly patients who may be more susceptible to adverse effects.
Renal_impairment
Contraindicated in severe renal impairment (creatinine clearance < 30 mL/min). Dose reduction may be necessary in moderate renal impairment.
How to Take
Oral administration, usually with or after food to minimize gastrointestinal discomfort.
Mechanism of Action
Nimesulide acts primarily as a preferential inhibitor of cyclooxygenase-2 (COX-2), an enzyme involved in the synthesis of prostaglandins. By inhibiting COX-2, it reduces the production of inflammatory prostaglandins, thereby exerting its anti-inflammatory, analgesic, and antipyretic effects.

Onset
Analgesic effects typically begin within 30 minutes to 1 hour.
Excretion
Primarily excreted in the urine (about 50%) and feces (about 29%) after metabolism.
Half life
The plasma elimination half-life is approximately 2 to 5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9. The main metabolite is 4'-hydroxy-nimesulide, which is also pharmacologically active.
Side Effects
Contraindications

Corticosteroids

SSRIs (Selective Serotonin Reuptake Inhibitors)
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose may include drowsiness, lethargy, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Rare cases of hypertension, acute renal failure, anaphylactoid reactions, and respiratory depression have been reported. Management involves symptomatic and supportive treatment. Gastric lavage and/or activated charcoal may be considered if ingested recently. There is no specific antidote.
Pregnancy & Lactation
Nimesulide is contraindicated during the third trimester of pregnancy due to potential adverse effects on the fetus (e.g., premature closure of the ductus arteriosus). It is generally not recommended during other trimesters or during lactation, as it may pass into breast milk and pose risks to the infant.
