Overview of the medicine
Nixamid 500 mg Tablet is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections including urinary tract infections, respiratory tract infections, skin and soft tissue infections, and gastrointestinal infections.
Uses & Indications
Dosage
Adults
500 mg once daily or 250 mg twice daily, depending on the severity and type of infection. Treatment duration typically 7-14 days.
Elderly
No specific dosage adjustment needed unless severe renal impairment is present. Monitor renal function.
Renal_impairment
Reduce dose in patients with severe renal impairment (creatinine clearance <30 mL/min). For example, 250 mg once daily.
How to Take
Take the tablet orally, with or without food. Swallow whole with water. Do not crush or chew.
Mechanism of Action
Nixafloxacin acts by inhibiting bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA, resulting in bacterial cell death.

Onset
Within 1-2 hours of administration.
Excretion
Mainly excreted unchanged in urine (approximately 50-70%) and feces (approximately 20-30%).
Half life
Approximately 6-8 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract; bioavailability approximately 90%. Peak plasma concentrations achieved within 1-2 hours.
Metabolism
Primarily hepatic metabolism to minor active and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to nixafloxacin or other fluoroquinolones
- History of tendon disorders related to fluoroquinolone use
- Children and adolescents (under 18 years of age) due to risk of arthropathy
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing the risk of bleeding. Monitor INR closely.
Corticosteroids
Concomitant use may increase the risk of tendon rupture.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Concomitant use may increase the risk of CNS stimulation and convulsive seizures.
Antacids (containing aluminum or magnesium), sucralfate, iron supplements, zinc supplements
May reduce the absorption of Nixamid. Administer Nixamid at least 2 hours before or 6 hours after these agents.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be performed. Provide symptomatic and supportive treatment. Monitor ECG due to potential for QTc prolongation. Maintain adequate hydration.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Not recommended during breastfeeding as nixafloxacin is excreted into breast milk and may cause adverse effects in the infant.
