Description
Overview of the medicine
Noctin 5 mg Tablet contains Zopiclone, a non-benzodiazepine hypnotic used for the short-term treatment of insomnia. It helps in falling asleep faster and reducing awakenings during the night, thereby improving sleep quality.
Uses & Indications
Dosage
Adults
The usual adult dose is 5 mg to 7.5 mg taken orally just before bedtime. Treatment duration should be as short as possible, usually ranging from a few days to a maximum of 4 weeks, including tapering off period.
Elderly
A lower dose of 3.75 mg at bedtime is recommended. The dose may be increased to 5 mg if clinically necessary.
Renal_impairment
A lower initial dose of 3.75 mg at bedtime is recommended. Use with caution.
How to Take
Take the tablet orally with water immediately before bedtime. Do not chew or crush the tablet. Ensure you have at least 7-8 hours available for sleep after taking the dose.
Mechanism of Action
Zopiclone binds to the GABA-A receptor complex at a site distinct from benzodiazepines but functionally coupled to it. This binding enhances the inhibitory effects of gamma-aminobutyric acid (GABA), leading to central nervous system depression, which manifests as sedative, hypnotic, anxiolytic, anticonvulsant, and muscle relaxant properties.

Mainly through urine (approximately 80%) as metabolites and feces (approximately 16%).

Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzymes (CYP3A4 and CYP2C8) to active (N-desmethylzopiclone) and inactive metabolites.

Drug Interactions
CNS Depressants (e.g., alcohol, opioids, other hypnotics, antidepressants)
Increased central depressant effect, risk of excessive sedation and respiratory depression.
CYP3A4 Inducers (e.g., rifampicin, carbamazepine, phenobarbital, phenytoin, St. John's wort)
May decrease Zopiclone plasma concentrations and reduce its efficacy.
CYP3A4 Inhibitors (e.g., erythromycin, clarithromycin, ketoconazole, itraconazole, ritonavir, grapefruit juice)
May increase Zopiclone plasma concentrations and enhance its effects.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose typically include varying degrees of central nervous system depression, ranging from drowsiness to coma, depending on the amount ingested. In mild cases, symptoms include somnolence, confusion, and lethargy. In more severe cases, ataxia, hypotonia, hypotension, methaemoglobinemia, respiratory depression, and coma may occur. Management is primarily supportive, including gastric lavage (if early), activated charcoal, and monitoring vital signs. Flumazenil may be considered as an antidote, but caution is advised in patients on chronic benzodiazepine/Z-drug therapy due to risk of withdrawal seizures.
Pregnancy & Lactation
Pregnancy Category C. Zopiclone should not be used during pregnancy unless the potential benefit outweighs the potential risk to the fetus. If used in late pregnancy, neonatal withdrawal symptoms or 'floppy infant syndrome' may occur. Zopiclone is excreted in breast milk; therefore, it is not recommended for use during lactation.
