Noficon 200 mg Capsule contains Fluconazole, an antifungal medication used to treat various fungal infections, including vaginal candidiasis, oral thrush, and systemic candidal infections.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: 150 mg single oral dose. For oropharyngeal candidiasis: 50-100 mg once daily for 7-14 days. For systemic infections: 200-400 mg once daily, duration based on infection type and severity.
Elderly
Dosage adjustments are generally not necessary in elderly patients unless there is evidence of renal impairment.
Renal_impairment
In patients with creatinine clearance <50 mL/min, the dose should be reduced by 50% after a normal initial loading dose.
How to Take
Noficon capsules can be taken orally, with or without food. Swallow the capsule whole with a glass of water. Do not chew or crush.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450-dependent 14α-demethylation of lanosterol, an essential step in fungal ergosterol biosynthesis. This leads to an accumulation of 14α-methyl sterols and a decrease in ergosterol, disrupting the fungal cell membrane integrity and function.

Clinical effects usually begin within 1-2 days, though full resolution of symptoms may take longer depending on the infection type.
Excretion
The primary route of elimination is renal, with approximately 80% of the dose excreted unchanged in the urine.

The plasma elimination half-life is approximately 30 hours, allowing for once-daily dosing for many indications.
Absorption
Fluconazole is well absorbed after oral administration, with systemic bioavailability typically exceeding 90%. Peak plasma concentrations are achieved within 0.5-1.5 hours.
Metabolism
Fluconazole is minimally metabolized (approximately 11% to 15% of the dose is metabolized to less active or inactive metabolites) in humans.
Side Effects
Contraindications
- Hypersensitivity to fluconazole, other azole derivatives, or any component of the formulation.
- Concomitant administration of terfenadine, cisapride, astemizole, pimozide, or quinidine with fluconazole (due to risk of QTc prolongation and serious cardiac arrhythmias).
Drug Interactions
Warfarin
Increased prothrombin time, monitor INR.
Phenytoin
Increased phenytoin serum concentrations, monitor levels.
Rifampicin
Decreased fluconazole plasma concentrations.
Cyclosporine, Tacrolimus
Increased plasma concentrations of immunosuppressants.
Oral hypoglycemics (sulfonylureas)
Increased plasma concentrations of sulfonylureas, risk of hypoglycemia.
Storage
Store in a dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Gastric lavage may be considered. Hemodialysis significantly reduces plasma levels of fluconazole and may be beneficial.
Pregnancy & Lactation
Fluconazole is generally avoided in pregnancy, especially in the first trimester, due to potential risk of congenital malformations with chronic high doses. It is excreted into breast milk; therefore, caution is advised during lactation.
