Description
Overview of the medicine
Nokof-DS 250 mg Syrup is a mucolytic agent containing Ambroxol Hydrochloride. It helps in thinning and loosening phlegm, making it easier to cough up and clear from the airways. It is used to relieve productive cough associated with various respiratory conditions.
Uses & Indications
Dosage
Adults
Adults and children over 12 years: 10 ml (2 teaspoonfuls) twice daily. The dose may be doubled for the first 2-3 days in severe conditions.
Elderly
No special dosage adjustment is generally required for the elderly, but caution is advised in patients with severe renal or hepatic impairment.
Renal_impairment
In severe renal impairment, the dose should be reduced or the dosage interval extended. Consult a physician.
How to Take
Take orally with or without food. Measure the dose using a standard measuring spoon or cup provided with the syrup.
Mechanism of Action
Ambroxol acts as a mucolytic by increasing serous mucus secretion in the respiratory tract, thereby thinning the phlegm and improving mucociliary clearance. It also activates depolymerization of mucopolysaccharides, further reducing mucus viscosity.
Pharmacokinetics
Onset
Within 30 minutes to 2 hours.
Excretion
Mainly excreted renally (approximately 90%) as metabolites, with a small portion excreted unchanged.
Half life
Approximately 7-12 hours.
Absorption
Rapidly and almost completely absorbed after oral administration, with peak plasma concentrations achieved within 1-2.5 hours.
Metabolism
Primarily metabolized in the liver to various inactive metabolites, mainly by conjugation.
Side Effects
Contraindications
- Hypersensitivity to Ambroxol or any component of the formulation.
- Patients with gastric or duodenal ulcers.
Drug Interactions
Antitussives (e.g., Codeine, Dextromethorphan)
Concomitant use with antitussives is not recommended as it may lead to a dangerous accumulation of secretions due to the cough reflex suppression.
Antibiotics (e.g., Amoxicillin, Cefuroxime, Erythromycin)
Ambroxol may increase the penetration of antibiotics into bronchial secretions, enhancing their effectiveness.
Storage
Store below 30°C in a dry place, away from direct sunlight. Keep out of reach of children.
Overdose
Symptoms of overdose include transient restlessness and diarrhea. Management is primarily symptomatic and supportive. Gastric lavage and induced emesis are not usually necessary. Keep the patient well hydrated.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy, especially in the first trimester, should be avoided unless the potential benefits outweigh the possible risks. Ambroxol is excreted in breast milk, so caution should be exercised when administered to a nursing mother.
