Description
Overview of the medicine
Normoglobin 25 gm Injection is an intravenous iron preparation used to treat iron deficiency anemia in patients who cannot tolerate or respond to oral iron therapy, or who have significant ongoing blood loss. It contains an iron-carbohydrate complex that releases elemental iron for hemoglobin synthesis.
Uses & Indications
Dosage
Adults
Total cumulative dose calculated based on body weight and target hemoglobin levels. Typically, 100 mg elemental iron administered up to 2-3 times per week, or larger single doses (e.g., 500 mg or 1000 mg) for total dose infusion, diluted in saline over 1-6 hours.
Elderly
Similar to adult dosage, but caution with cardiovascular status.
Renal_impairment
Dosing as per adults, frequently used in dialysis patients.
How to Take
Administered by slow intravenous infusion after dilution in 0.9% sodium chloride solution. A test dose may be required for certain formulations (e.g., high molecular weight iron dextran).
Mechanism of Action
Iron (III) hydroxide dextran complex is taken up by the reticuloendothelial system (RES) cells, where iron is released from the complex. This elemental iron then binds to transferrin and is transported to the bone marrow for erythroblast incorporation into hemoglobin, and to storage sites like ferritin.
Pharmacokinetics
Onset
Hemoglobin increase usually observed within 2-4 weeks.
Excretion
Primarily via the reticuloendothelial system and incorporation into red blood cells. Minimal renal excretion.
Half life
The iron dextran complex has a half-life of 3-4 days in plasma, but iron utilization extends over weeks.
Absorption
Administered intravenously, thus bioavailability is 100%. Iron is slowly released from the complex.
Metabolism
The dextran component is metabolized or excreted. Iron is recycled and utilized by the body.
Side Effects
Contraindications
- Known hypersensitivity to iron dextran or any component of the formulation.
- All anemias not caused by iron deficiency.
- Iron overload or disturbances in iron utilization.
- Acute phase of infectious diseases.
Drug Interactions
ACE Inhibitors
May increase the risk of systemic reactions, especially in patients with inflammatory conditions.
Oral Iron Preparations
Concomitant administration of oral iron preparations reduces absorption of oral iron. Oral iron should be started only after at least 5 days following the last injection of Normoglobin.
Storage
Store below 25°C. Do not freeze. Protect from light.
Overdose
Iron overload can occur. Treatment involves symptomatic and supportive measures, and chelation therapy with deferoxamine in severe cases.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and potential benefit outweighs the risk to the fetus. Limited data on excretion in breast milk; caution advised.
