Description
Overview of the medicine
Novarin SR (Diclofenac Sodium Sustained Release) is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and stiffness caused by arthritis and other inflammatory conditions. The sustained-release formulation allows for once-daily dosing.
Uses & Indications
Dosage
Adults
Typically 100 mg once daily, preferably taken with food.
Elderly
Start with the lowest effective dose; monitor for adverse effects.
Renal_impairment
Use with caution; dose reduction may be necessary in severe impairment. Not recommended for severe renal failure.
How to Take
Take orally, preferably with or after food to minimize gastrointestinal upset. Swallow the tablet whole; do not crush, chew, or divide.
Mechanism of Action
Diclofenac inhibits cyclooxygenase (COX-1 and COX-2) enzymes, which are involved in the synthesis of prostaglandins. By reducing prostaglandin production, it decreases inflammation, pain, and fever.
Pharmacokinetics
Onset
Pain relief may begin within 1-2 hours; full anti-inflammatory effect may take longer.
Excretion
Approximately 60% renally excreted as metabolites, and 30-40% via bile and feces.
Half life
Approximately 1-2 hours for the parent compound, but sustained release prolongs therapeutic effects.
Absorption
Well absorbed orally; peak plasma levels achieved 2-4 hours after administration of sustained-release formulation.
Metabolism
Primarily hepatic metabolism via cytochrome P450 isoenzymes (mainly CYP2C9), leading to multiple hydroxylated and methoxylated metabolites.
Side Effects
Contraindications
- Hypersensitivity to diclofenac or other NSAIDs
- Active gastrointestinal bleeding or peptic ulcer
- Severe heart failure, renal or hepatic impairment
- Third trimester of pregnancy
- History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
ACE inhibitors and diuretics
Reduced antihypertensive and diuretic effects.
Warfarin and other anticoagulants
Increased risk of bleeding.
SSRIs (Selective Serotonin Reuptake Inhibitors)
Increased risk of GI bleeding.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management is supportive and symptomatic; gastric lavage or activated charcoal may be considered if ingestion was recent.
Pregnancy & Lactation
Avoid during the third trimester of pregnancy due to risk of premature closure of the ductus arteriosus. Use with caution in the first and second trimesters only if the potential benefit outweighs the risk. Diclofenac is excreted in breast milk; avoid during lactation.
