Description
Overview of the medicine
Levofloxacin is a fluoroquinolone antibiotic used to treat a wide range of bacterial infections, including respiratory tract, skin, urinary tract, and certain other infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For mild to moderate infections: 200 mg once daily. For severe infections, higher doses (500-750 mg) may be prescribed.
Elderly
Dosage adjustment may be necessary based on renal function. Monitor carefully.
Renal_impairment
Dosage adjustment is required for patients with creatinine clearance less than 50 mL/min. Refer to specific guidelines.
How to Take
Novetor tablet can be taken orally with or without food. It should be swallowed whole with a glass of water. Do not crush or chew. Ensure adequate hydration during treatment.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to cell death.
Pharmacokinetics
Onset
Varies depending on the type and severity of infection, typically within hours to days.
Excretion
Primarily excreted unchanged via the kidneys, mainly by glomerular filtration and tubular secretion. Renal clearance accounts for about 70% of total body clearance.
Half life
Approximately 6-8 hours, enabling once-daily dosing.
Absorption
Rapid and almost complete oral absorption; absolute bioavailability is approximately 99%. Peak plasma concentrations are usually reached within 1-2 hours.
Metabolism
Minimally metabolized in the liver; less than 5% of a dose is metabolized.
Side Effects
Contraindications
- Hypersensitivity to levofloxacin, other fluoroquinolones, or any excipients
- History of tendinitis or tendon rupture associated with fluoroquinolone use
- Epilepsy or other seizure disorders
- Children and adolescents below 18 years of age
Drug Interactions
Warfarin
Increased anticoagulant effect, potentially leading to bleeding. INR monitoring advised.
Corticosteroids
Increased risk of tendinitis and tendon rupture.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Increased risk of CNS stimulation and convulsive seizures.
Antacids, iron supplements, sucralfate, zinc supplements
Reduced absorption of levofloxacin. Should be taken at least 2 hours before or 2 hours after levofloxacin.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include confusion, dizziness, impaired consciousness, convulsive seizures, and cardiac reactions such as QT prolongation. Management involves gastric lavage, symptomatic treatment, and supportive measures. ECG monitoring is essential due to the risk of QT prolongation.
Pregnancy & Lactation
Not recommended during pregnancy and lactation due to potential adverse effects on the fetus/infant (e.g., cartilage damage). Use only if the potential benefit justifies the potential risk to the fetus/infant and under strict medical supervision.

YesClinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of levofloxacin in treating various bacterial infections. Post-marketing surveillance continues to monitor for rare adverse events.
Lab Monitoring
- Renal function tests (especially in elderly or renally impaired patients)
- Blood glucose levels (especially in diabetic patients, due to risk of dysglycemia)
- ECG monitoring (in patients at risk for QT prolongation)
Doctor Notes
- Educate patients thoroughly about the black box warnings, especially regarding tendinitis and tendon rupture. Advise immediate reporting of any musculoskeletal pain.
- Assess renal function prior to and during treatment, especially in elderly patients, to adjust dosage as needed.
- Counsel diabetic patients on the risk of dysglycemia (hypoglycemia and hyperglycemia) and recommend blood glucose monitoring.
Patient Guidelines
- Complete the entire course of medication as prescribed, even if symptoms improve.
- Do not share your medication with others.
- Report any unusual pain, swelling, or tenderness in joints or tendons immediately.
- Avoid excessive sun exposure, as levofloxacin can increase sensitivity to sunlight.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Novetor may cause dizziness, lightheadedness, or visual disturbances. Patients should be cautioned about operating hazardous machinery, including automobiles, until they are reasonably certain that levofloxacin does not adversely affect them.
Lifestyle Advice
- Maintain adequate hydration throughout the treatment period.
- Avoid alcohol consumption during the course of treatment, as it may exacerbate some side effects.
