Description
Overview of the medicine
Novomix 30 is a pre-mixed insulin analogue containing 30% soluble insulin aspart and 70% protamine-crystallized insulin aspart. It is used for the treatment of diabetes mellitus in adults, adolescents, and children 10 years and above.
Uses & Indications
Dosage
Adults
Dosage is individualized based on blood glucose levels and patient's metabolic needs. Typically administered once or twice daily, immediately before a main meal (within 0-10 minutes).
Elderly
Dosage should be individualized and carefully monitored due to potential for hypoglycemia and age-related decline in renal function.
Renal_impairment
Dosage adjustment may be required; careful monitoring of glucose levels is essential due to increased risk of hypoglycemia.
How to Take
Administer by subcutaneous injection into the thigh, abdominal wall, upper arm, or deltoid region. Rotate injection sites within the same region to reduce the risk of lipodystrophy. Administer immediately before a meal (within 0-10 minutes).
Mechanism of Action
Insulin lowers blood glucose by stimulating peripheral glucose uptake, especially by skeletal muscle and fat, and by inhibiting hepatic glucose production. Insulin inhibits lipolysis and proteolysis, and enhances protein synthesis.
Pharmacokinetics
Onset
10-20 minutes
Excretion
Primarily via renal excretion.
Half life
Approximately 5-8 hours (apparent half-life)
Absorption
Rapidly absorbed after subcutaneous injection. Peak concentration reached within 1.5-2.5 hours.
Metabolism
Primarily metabolized by insulin degrading enzymes in liver and kidney.
Side Effects
Contraindications
- Hypersensitivity to insulin aspart or any of the excipients.
- Episodes of hypoglycemia.
Drug Interactions
Beta-blockers
May mask symptoms of hypoglycemia and delay recovery from it.
ACE inhibitors
May increase the hypoglycemic effect of insulin.
Corticosteroids
May decrease the hypoglycemic effect of insulin, requiring dose adjustment.
Thiazide diuretics
May decrease the hypoglycemic effect of insulin.
Oral Contraceptives
May decrease the hypoglycemic effect of insulin.
Storage
Unopened: Store in a refrigerator (2°C - 8°C). Do not freeze. In-use: Store below 30°C for up to 4 weeks. Do not refrigerate pens in use. Keep the pen cap on to protect from light.
Overdose
Severe hypoglycemia, which may be treated by oral glucose if conscious, or by intravenous glucose or glucagon injection if unconscious. Medical supervision is required.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation, but close monitoring of blood glucose is essential. Consult doctor to adjust insulin regimen if necessary.

WHO Essential Medicine
YesClinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of Novomix 30 in managing blood glucose levels in patients with type 1 and type 2 diabetes mellitus.
Lab Monitoring
- Regular monitoring of blood glucose levels (fasting, pre-meal, post-prandial).
- HbA1c (glycated hemoglobin) levels every 3-6 months.
- Renal function (creatinine, GFR) in patients with kidney impairment.
Doctor Notes
- Emphasize the importance of patient education regarding proper injection technique, site rotation, and recognition/management of hypoglycemia.
- Regular monitoring of HbA1c and self-monitoring of blood glucose (SMBG) are crucial for optimal glycemic control.
- Patients should be advised to always carry a source of fast-acting glucose.
Patient Guidelines
- Always carry identification indicating you have diabetes.
- Never share insulin pens or needles with other people, even if the needle is changed.
- Learn to recognize the symptoms of hypoglycemia (e.g., sweating, trembling, dizziness, hunger) and hyperglycemia (e.g., increased thirst, frequent urination).
- Monitor your blood glucose regularly as advised by your doctor.
Missed Dose Advice
If a dose is missed, blood glucose levels should be checked. Do not take a double dose or an extra dose later in the day to compensate for the missed dose, as this may lead to severe hypoglycemia. Consult a doctor or pharmacist for advice.
Driving Precautions
Patients should be advised about the risk of hypoglycemia and the need for caution while driving or operating machinery. Blood glucose should be checked before driving, especially if symptoms of hypoglycemia are present.
Lifestyle Advice
- Maintain a balanced diet and engage in regular physical activity as part of your diabetes management plan.
- Avoid skipping meals and ensure consistency in meal timing and carbohydrate intake.
- Limit alcohol consumption.
