Overview of the medicine
Nucef-CV 250 mg Tablet is a combination antibiotic used to treat a wide range of bacterial infections. It contains Cefixime, a third-generation cephalosporin, which fights bacteria by inhibiting cell wall synthesis, and Clavulanic Acid, a beta-lactamase inhibitor, which prevents bacterial enzymes from destroying Cefixime.

Dosage
Adults
The usual adult dose is Cefixime 200 mg + Clavulanic Acid 125 mg, orally twice daily. For Nucef-CV 250 mg Tablet, consult a physician for specific dosage, as it's an atypical strength for a combination with clavulanic acid. Typically, 250mg Cefixime would be combined with 62.5mg or 125mg Clavulanic Acid, taken twice daily.
Elderly
No specific dose adjustment is required in elderly patients with normal renal function. However, renal function should be monitored.
Renal_impairment
For patients with creatinine clearance <60 mL/min, dosage adjustment is necessary. For creatinine clearance between 20-40 mL/min, reduce dose by half or increase dosing interval. For <20 mL/min, further reduction is needed.
How to Take
Take the tablet orally with or without food, preferably at the same time each day, as directed by your doctor. Swallow the tablet whole with water; do not crush, chew, or break it.
Mechanism of Action
Cefixime binds to penicillin-binding proteins (PBPs) in the bacterial cell wall, inhibiting peptidoglycan synthesis, which leads to bacterial cell lysis and death. Clavulanic Acid, a beta-lactamase inhibitor, irreversibly binds to and inactivates beta-lactamase enzymes produced by bacteria, thus protecting Cefixime from degradation and extending its spectrum of activity.

Onset
Within 1-2 hours for Cefixime; Clavulanic acid peak plasma levels usually reached within 1 hour.
Excretion
Cefixime: Primarily excreted unchanged in urine via glomerular filtration and tubular secretion. Clavulanic Acid: Excreted renally as metabolites and unchanged drug.
Half life
Cefixime: 3-4 hours. Clavulanic Acid: Approximately 1 hour.
Absorption
Cefixime: Approximately 40-50% orally absorbed. Clavulanic Acid: Readily absorbed from the gastrointestinal tract.
Metabolism
Cefixime: Minimally metabolized. Clavulanic Acid: Extensively metabolized.
Side Effects
Contraindications
- Known hypersensitivity to cefixime, clavulanic acid, any other cephalosporin antibiotics, or penicillin antibiotics.
- History of severe hypersensitivity reactions to beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase cefixime plasma concentrations.
Probenecid
May increase cefixime plasma concentrations by decreasing renal tubular secretion.
Carbamazepine
Cefixime may increase carbamazepine levels.
Anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect, leading to an increased risk of bleeding. Monitor INR closely.

Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be given. Gastric lavage may be indicated. Hemodialysis or peritoneal dialysis does not remove significant quantities of cefixime or clavulanic acid from the body.
