Description
Overview of the medicine
Nucip 500 mg Tablet is an antibiotic used to treat a wide range of bacterial infections including infections of the urinary tract, respiratory tract, skin, bones, joints, and gastrointestinal tract. It belongs to the fluoroquinolone class of antibiotics.
Uses & Indications
Dosage
Adults
Typically 250-750 mg orally twice daily, depending on the type and severity of infection. Treatment duration varies.
Elderly
No specific dosage adjustment based on age alone; adjust based on renal function if impaired.
Renal_impairment
Dosage reduction is necessary for patients with significant renal impairment. Refer to specific guidelines based on creatinine clearance.
How to Take
Take the tablet orally with a glass of water, with or without food. Do not chew or crush. Avoid taking with dairy products or calcium-fortified juices alone. Maintain adequate hydration.
Mechanism of Action
Ciprofloxacin, a fluoroquinolone, inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Primarily excreted renally (approximately 50-70% as unchanged drug) and to a lesser extent via bile and feces.
Half life
Approximately 3-5 hours (in individuals with normal renal function)
Absorption
Rapidly and well absorbed from the gastrointestinal tract; absolute bioavailability is approximately 70%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Partially metabolized in the liver to four active metabolites (desethyleneciprofloxacin, sulfociprofloxacin, oxociprofloxacin, and formylciprofloxacin), but these have less antimicrobial activity than the parent drug.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- Concomitant administration with tizanidine due to increased risk of hypotension and sedation.
- History of quinolone-associated tendinopathy or tendon rupture.
Drug Interactions
Warfarin
Potentiates the anticoagulant effects, requiring INR monitoring.
Tizanidine
Contraindicated due to significant increase in tizanidine plasma concentrations, leading to hypotension and sedation.
Theophylline
Increased plasma concentrations of theophylline, leading to increased risk of adverse reactions.
Corticosteroids
Increased risk of tendinopathy and tendon rupture.
Antacids (containing magnesium, aluminum, calcium), Iron, Zinc supplements
Decreased absorption of ciprofloxacin if taken concurrently. Administer ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric emptying (e.g., by inducing vomiting or gastric lavage) may be considered. Symptomatic treatment and supportive measures should be implemented. Adequate hydration should be maintained. Hemodialysis or peritoneal dialysis remove only a small amount of ciprofloxacin from the body.
Pregnancy & Lactation
Ciprofloxacin is generally not recommended during pregnancy or breastfeeding due to potential risks to the fetus/infant (e.g., cartilage damage). Use only if the potential benefit outweighs the potential risk and no safer alternatives are available. Consult a healthcare provider.

Lab Monitoring
- Renal function tests (especially in elderly patients or those with pre-existing renal impairment)
- Liver function tests (periodically, especially with long-term therapy)
- Complete blood count (rarely, but for monitoring bone marrow suppression with prolonged use)
Doctor Notes
- Emphasize the black box warnings regarding tendinopathy, peripheral neuropathy, and CNS effects.
- Advise patients about potential drug interactions, especially with tizanidine, theophylline, and warfarin.
- Educate on completing the full course and avoiding dairy/antacids around dosing times.
Patient Guidelines
- Complete the full course of antibiotic, even if symptoms improve, to prevent resistance.
- Drink plenty of fluids to prevent crystallization in the urine.
- Avoid excessive sun exposure and use protective clothing/sunscreen due to risk of photosensitivity.
- Report any joint pain, swelling, or unusual sensations immediately to your doctor.
- Do not take with antacids containing magnesium or aluminum, iron or zinc supplements, or dairy products/calcium-fortified juices without consulting your doctor.
