Description
Overview of the medicine
Nutryelt Injection is a sterile solution containing a balanced combination of essential vitamins and trace elements, formulated for intravenous administration to patients requiring parenteral nutritional support. It helps to meet the daily requirements of vital micronutrients when oral intake is inadequate or contraindicated.
Uses & Indications
Dosage
Adults
The usual adult dose is 10 ml daily, administered as a slow intravenous infusion, typically added to a larger volume of parenteral nutrition solution. The specific dose may vary based on the patient's nutritional status, clinical condition, and body weight.
Elderly
No specific dose adjustment is usually required for elderly patients, but renal and hepatic function should be monitored, and doses adjusted if necessary.
Renal_impairment
Use with caution in patients with severe renal impairment, as some trace elements (e.g., manganese) and water-soluble vitamins may accumulate. Dosage adjustment or closer monitoring of plasma levels may be necessary.
How to Take
Nutryelt Injection is for intravenous administration only. It should be diluted in a suitable intravenous infusion solution (e.g., glucose, saline, or amino acid solutions) before use and administered as a slow infusion over several hours. It should not be administered as a direct bolus injection.
Mechanism of Action
Nutryelt provides a balanced supply of essential water-soluble and fat-soluble vitamins, along with vital trace elements (e.g., zinc, copper, manganese, selenium, chromium) that act as co-factors for various enzymatic reactions, maintain cellular integrity, and support overall metabolic functions. These nutrients are crucial for energy production, immune function, wound healing, and erythropoiesis.
Pharmacokinetics
Onset
Immediate, with components being rapidly distributed to tissues.
Excretion
Excretion primarily occurs via renal (for water-soluble vitamins and some trace elements) and biliary (for fat-soluble vitamins and other trace elements) routes. Some are stored in tissues.
Half life
Varies significantly for individual vitamins and trace elements, ranging from minutes to days, depending on their metabolic fate and storage.
Absorption
Administered intravenously, hence bioavailability is 100%. The components are directly absorbed into the systemic circulation.
Metabolism
Individual vitamins and trace elements are metabolized via their respective pathways (e.g., phosphorylation of vitamins, protein binding of trace elements) in the liver and other tissues.
Side Effects
Contraindications
- •Known hypersensitivity to any component of the injection.
- •Patients with pre-existing hypervitaminosis (especially fat-soluble vitamins) or acute overdose of trace elements.
- •Severe hepatic or renal impairment without appropriate monitoring.
- •Patients with impaired bile secretion.
Drug Interactions
Levodopa
Pyridoxine (Vitamin B6) may accelerate the peripheral metabolism of levodopa, reducing its efficacy, particularly when given without a peripheral dopa decarboxylase inhibitor.
Tetracyclines
Certain trace elements (e.g., iron, zinc) can chelate with tetracyclines, potentially reducing their absorption if administered orally. Minimal interaction expected with IV administration of Nutryelt.
Vitamin K antagonists (e.g., Warfarin)
Large doses of Vitamin K can counteract the effects of anticoagulants. Nutryelt contains a small, physiological amount of Vitamin K, unlikely to cause significant interaction.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Overdose symptoms may include hypervitaminosis (especially fat-soluble vitamins like A, D, E) or mineral toxicity (e.g., zinc, copper). Symptoms vary depending on the specific nutrient and may include nausea, vomiting, headache, fatigue, skin changes, neurological disturbances, or electrolyte imbalances. Management involves discontinuing the infusion, supportive care, and symptomatic treatment. Electrolyte and vitamin levels should be monitored.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Essential vitamins and trace elements are normally required during pregnancy and lactation. However, doses should be carefully monitored to avoid hypervitaminosis. Consult a doctor.
