Description
Overview of the medicine
Nyclobate NN Cream is a combination medication used to treat inflammatory skin conditions (like eczema, psoriasis, dermatitis) that are complicated by bacterial and/or fungal infections. It contains Clobetasol Propionate (a potent corticosteroid), Neomycin Sulfate (an antibiotic), and Nystatin (an antifungal).
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin areas once or twice daily. Do not use for more than 2 consecutive weeks.
Elderly
No specific dosage adjustment required, but use with caution due to thinner skin and increased potential for systemic absorption.
Renal_impairment
No specific dosage adjustment for topical use.
How to Take
For external use only. Apply a thin layer to the affected area and rub in gently. Wash hands thoroughly before and after application. Avoid contact with eyes, mouth, and other mucous membranes.
Mechanism of Action
Clobetasol Propionate reduces inflammation and itching by inhibiting phospholipase A2. Neomycin Sulfate is an aminoglycoside antibiotic that inhibits bacterial protein synthesis. Nystatin is a polyene antifungal that disrupts fungal cell membranes.
Pharmacokinetics
Onset
Anti-inflammatory effects of Clobetasol are generally observed within a few hours to days. Antibacterial and antifungal effects develop over several days of treatment.
Excretion
Clobetasol metabolites are excreted mainly by the kidneys and some by bile. Unabsorbed Neomycin and Nystatin are excreted unchanged.
Half life
Topical Clobetasol's systemic half-life is not precisely defined for topical application but generally short. Systemic absorption of Neomycin and Nystatin is too low to have a significant systemic half-life.
Absorption
Systemic absorption of Clobetasol Propionate occurs, especially with prolonged use, on large surface areas, or under occlusive dressings. Neomycin and Nystatin are minimally absorbed from intact skin.
Metabolism
Absorbed Clobetasol is primarily metabolized in the liver. Neomycin and Nystatin are poorly metabolized when applied topically.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate, Neomycin Sulfate, Nystatin, or any component of the formulation.
- Primary cutaneous viral infections (e.g., herpes simplex, varicella).
- Rosacea, perioral dermatitis, acne vulgaris, and primary fungal or bacterial skin infections not associated with inflammation.
- Not for use in children under 1 year of age.
Drug Interactions
General
No significant topical drug interactions are reported. Systemic interactions are unlikely due to minimal absorption but could theoretically occur with extensive prolonged use of Clobetasol (e.g., with CYP3A4 inhibitors).
Storage
Store in a cool, dry place (below 30°C). Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Excessive or prolonged use of topical corticosteroids can suppress pituitary-adrenal function, leading to secondary adrenal insufficiency and manifestations of Cushing's syndrome. Overdose management involves gradual withdrawal of the drug and symptomatic/supportive treatment.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy and lactation only if the potential benefit justifies the potential risk to the fetus/infant. Avoid large areas, prolonged use, or application under occlusive dressings. Consult a doctor.

Clinical Trials
As a well-established combination of generic drugs, specific large-scale clinical trials for Nyclobate NN itself are limited. The efficacy and safety are derived from individual component studies and extensive post-market experience.
Lab Monitoring
- HPA axis suppression test (e.g., ACTH stimulation test, morning plasma cortisol) may be considered in patients using the cream extensively or for prolonged periods.
- Blood glucose monitoring for patients at risk of hyperglycemia.
Doctor Notes
- Emphasize short-term use (max 2 weeks, especially on face/intertriginous areas).
- Educate patients on proper application technique and avoiding occlusive dressings.
- Be aware of potential for systemic absorption in children or with extensive use.
Patient Guidelines
- Use for external use only; do not ingest.
- Apply a thin layer to affected area(s) as directed by your doctor; do not cover with airtight dressings unless advised.
- Do not use for longer than the prescribed duration (typically short-term) to avoid adverse effects.
- Avoid contact with eyes and mucous membranes.
- Inform your doctor if your condition does not improve or worsens after treatment.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply extra medicine to make up for a missed dose.
