Description
Overview of the medicine
Nytenso 20 mg tablet contains Nytenartan, an angiotensin II receptor blocker (ARB), used to treat high blood pressure (hypertension) and heart failure. It helps to relax blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
The usual starting dose is 20 mg once daily. The dose may be increased to 40 mg once daily depending on the blood pressure response.
Elderly
No initial dosage adjustment is usually necessary, but lower starting doses may be considered for patients >75 years with significant renal impairment.
Renal_impairment
Initial dose of 10 mg once daily is recommended for patients with moderate to severe renal impairment. Careful monitoring is advised.
How to Take
Take Nytenso tablet orally with or without food, at the same time each day, preferably in the morning.
Mechanism of Action
Nytenartan selectively blocks the binding of angiotensin II to the AT1 receptor, preventing the vasoconstrictive and aldosterone-secreting effects of angiotensin II. This leads to vasodilation, decreased blood pressure, and reduced fluid retention.
Pharmacokinetics
Onset
Onset of antihypertensive effect within 1-2 hours.
Excretion
Primarily excreted via bile and urine, with about 30% excreted unchanged in urine.
Half life
Elimination half-life is approximately 6-9 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract with an oral bioavailability of approximately 30-40%.
Metabolism
Undergoes extensive hepatic metabolism, with some active metabolites.
Side Effects
Contraindications
- Hypersensitivity to Nytenartan or any component of the formulation
- Pregnancy (especially in the second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment
- History of angioedema related to previous ARB or ACE inhibitor therapy
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity have been reported with concomitant use of ARBs.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal function deterioration.
Potassium-sparing diuretics & Potassium supplements
May lead to increased serum potassium levels (hyperkalemia).
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension (low blood pressure) and tachycardia; bradycardia might occur. Management involves symptomatic and supportive treatment, including intravenous fluid administration.
Pregnancy & Lactation
Nytenso is contraindicated during pregnancy, especially in the second and third trimesters, due to risks of fetal injury and death. It is not recommended during lactation; evaluate the potential risks and benefits.
