Description
Overview of the medicine
Ocarnix 500 mg Oral Solution is a broad-spectrum macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and genitourinary infections. It works by inhibiting bacterial protein synthesis.
Uses & Indications
Dosage
Adults
Typically 500 mg once daily for 3-7 days, depending on the infection type and severity.
Elderly
No specific dose adjustment is generally required in elderly patients, but monitoring of renal and hepatic function is advised.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl < 10 mL/min).
How to Take
Ocarnix oral solution can be taken with or without food. Administer the full prescribed dose using the provided measuring device. Shake well before use.
Mechanism of Action
Ocarnix acts by binding to the 50S ribosomal subunit of susceptible bacteria, thereby inhibiting protein synthesis and consequently bacterial growth and replication. It is primarily bacteriostatic.
Pharmacokinetics
Onset
Onset of antibacterial activity typically within 1-2 hours of administration.
Excretion
Mainly excreted unchanged through biliary excretion; a minor portion is renally excreted.
Half life
Elimination half-life ranges from 12 to 18 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract; oral bioavailability is good and not significantly affected by food.
Metabolism
Primarily metabolized in the liver via cytochrome P450 enzymes (particularly CYP3A4).
Side Effects
Contraindications
- Known hypersensitivity to ocarnix, other macrolide antibiotics, or any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior macrolide use.
- Severe hepatic impairment.
Drug Interactions
Digoxin
May increase serum digoxin levels; monitor digoxin levels and adjust dose if necessary.
Warfarin
Increased anticoagulant effect (elevated INR) has been reported; monitor INR closely.
Nelfinavir
Concomitant use may increase Ocarnix serum concentrations.
Antacids (containing aluminum or magnesium)
May reduce the absorption of Ocarnix if taken concurrently. Administer Ocarnix at least 2 hours before or 4 hours after antacids.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Do not freeze. Keep out of sight and reach of children.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves gastric lavage, supportive care, and monitoring of vital signs. Hemodialysis is not effective in removing Ocarnix.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use Ocarnix during pregnancy only if clearly needed. Ocarnix is excreted in breast milk; caution should be exercised when administered to a nursing woman.
