Description
Overview of the medicine
Oxytocin is a hormone produced in the hypothalamus and released by the posterior pituitary gland. It is used to induce labor, augment contractions, prevent and treat postpartum hemorrhage, and to manage incomplete abortions.
Uses & Indications
Dosage
Adults
Induction/Augmentation of Labor: Initially 0.5-1 mIU/minute IV, gradually increased by 1-2 mIU/minute every 30-60 minutes until desired contraction pattern is established. Postpartum Hemorrhage: 10-40 IU in 1000 mL IV fluid, infused at a rate to control uterine atony, or 10 IU IM after delivery of placenta.
Elderly
Not typically indicated for elderly patients as its primary use is obstetric.
Renal_impairment
No specific dosage adjustment required; however, fluid and electrolyte balance should be carefully monitored due to potential antidiuretic effect.
How to Take
Administered intravenously (usually diluted as an infusion) or intramuscularly by a healthcare professional in a controlled clinical setting.
Mechanism of Action
Oxytocin stimulates uterine smooth muscle contractions by increasing the intracellular calcium concentration in myometrial cells, primarily through activation of G-protein coupled oxytocin receptors. It also causes contraction of myoepithelial cells in the mammary gland, leading to milk ejection.
Pharmacokinetics
Onset
IV: Immediate; IM: 3-5 minutes.
Excretion
Excreted renally, with a small amount excreted in bile.
Half life
Approximately 3-5 minutes (plasma half-life).
Absorption
Rapidly absorbed after intramuscular injection; administered intravenously for controlled effects.
Metabolism
Rapidly inactivated by oxytocinase (an enzyme found in plasma, especially high during pregnancy, and various tissues).
Side Effects
Contraindications
- Significant cephalopelvic disproportion.
- Unfavorable fetal position or presentation (e.g., transverse lie).
- Prior uterine surgery (e.g., classical C-section) or other conditions where vaginal delivery is contraindicated.
- Fetal distress where delivery is not imminent.
- Hypertonic uterine dysfunction or overdistended uterus.
- Placenta previa, vasa previa, or umbilical cord prolapse.
Drug Interactions
Vasopressors
Concomitant use with oxytocin may lead to severe hypertension, especially if vasopressors are administered postpartum or with caudal block anesthesia.
Prostaglandins
Concurrent use may enhance the uterotonic effect, increasing the risk of uterine hyperstimulation.
Inhalation Anesthetics
May decrease the uterotonic effect of oxytocin.
Cyclopropane Anesthetics
May increase the risk of cardiac arrhythmias.
Storage
Store between 2°C to 8°C (refrigerate). Protect from light. Do not freeze.
Overdose
Overdose can lead to uterine hypertonicity, tetanic contractions, fetal bradycardia, fetal distress, and potentially uterine rupture. Management involves immediate discontinuation of oxytocin, maternal repositioning (left lateral), oxygen administration, and if severe, administration of a tocolytic agent.
Pregnancy & Lactation
Pregnancy: Used during pregnancy for specific indications related to labor and delivery. Lactation: Excreted in small amounts into breast milk; generally considered safe for use during lactation when medically indicated, but caution is advised.
