Description
Overview of the medicine
Oclazid MR is an oral antidiabetic drug (sulfonylurea class) used to control blood sugar levels in people with type 2 diabetes mellitus. It helps the pancreas produce more insulin and lowers blood glucose.
Uses & Indications
Dosage
Adults
Initial dose is usually 30 mg once daily, taken with breakfast. Dose may be increased in steps to a maximum of 120 mg daily.
Elderly
Similar to adult dose, with careful monitoring. Start with lower dose if needed.
Renal_impairment
Mild to moderate renal impairment: standard dosing with careful monitoring. Severe renal impairment: not recommended.
How to Take
Take orally once daily with breakfast. Swallow the tablet whole with water; do not chew or crush.
Mechanism of Action
Gliclazide stimulates insulin release from pancreatic beta cells by binding to the sulfonylurea receptor 1 (SUR1) on the beta-cell membrane, leading to closure of ATP-sensitive potassium channels. This causes depolarization, calcium influx, and subsequent insulin secretion.
Pharmacokinetics
Onset
Onset of action within 2 hours, sustained effect due to MR.
Excretion
Mainly excreted in urine (less than 1% as unchanged drug), small amount in feces.
Half life
Approximately 12-20 hours for MR formulation.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma levels achieved within 6-12 hours for MR formulation.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, into inactive metabolites.
Side Effects
Contraindications
- •Type 1 diabetes mellitus
- •Diabetic ketoacidosis
- •Severe renal or hepatic impairment
- •Hypersensitivity to gliclazide or other sulfonylureas
- •Concomitant use with miconazole (systemic)
Drug Interactions
Miconazole
Potentiates hypoglycemic effect, severe hypoglycemia.
Beta-blockers
May mask symptoms of hypoglycemia.
Phenylbutazone
Increases hypoglycemic effect.
Corticosteroids, Diuretics, Estrogens
May reduce hypoglycemic effect (hyperglycemic agents).
ACE inhibitors, Fluconazole, H2 antagonists
May enhance hypoglycemic effect.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypoglycemia. Management involves immediate glucose administration (oral or IV), monitoring blood glucose, and supportive care.
Pregnancy & Lactation
Not recommended during pregnancy (insulin is preferred). Avoid during lactation as it may excrete into breast milk.
