Overview of the medicine
Ocutrop-Plus eye drop is a combination of Moxifloxacin (a fluoroquinolone antibiotic) and Dexamethasone (a corticosteroid). It is used to treat bacterial eye infections where inflammation is also present, providing both antibacterial action and anti-inflammatory relief.
Uses & Indications
Dosage
Adults
Instill 1 drop in the affected eye(s) 2-4 times daily. The duration of treatment is usually 5-7 days, or as directed by the physician.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required for patients with renal impairment, as systemic absorption from ophthalmic use is minimal.
How to Take
For ophthalmic use only. Tilt the head back, pull down the lower eyelid to form a pouch, and instill the drop into the pouch. Do not touch the dropper tip to the eye or any other surface to avoid contamination. After instilling the drop, gently close the eye for a few minutes.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death. Dexamethasone is a potent synthetic corticosteroid that suppresses the inflammatory response by inhibiting various inflammatory mediators and cells.
Pharmacokinetics
Onset
Onset of action is relatively rapid, with symptomatic relief often noted within hours for inflammation and 1-2 days for infection.
Excretion
Systemically absorbed moxifloxacin is excreted via renal and fecal routes. Dexamethasone and its metabolites are primarily excreted by the kidneys.
Half life
The systemic half-life of moxifloxacin is approximately 11-15 hours, and for dexamethasone, it is 3-4 hours. However, local ocular half-life is significantly shorter due to tear film turnover and drainage.
Absorption
Moxifloxacin and Dexamethasone show very low systemic absorption following ophthalmic administration, resulting in high local concentrations in ocular tissues. Peak aqueous humour concentration is achieved within 30-60 minutes.
Metabolism
Systemically absorbed moxifloxacin is metabolized in the liver. Dexamethasone is also primarily metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to Moxifloxacin, Dexamethasone, or any other component of the formulation
- •Viral diseases of the cornea and conjunctiva (e.g., epithelial herpes simplex keratitis, vaccinia, varicella)
- •Mycobacterial infections of the eye
- •Fungal diseases of ocular structures
Drug Interactions
Other topical ophthalmic preparations
If more than one topical ophthalmic drug is being used, they should be administered at least 5 minutes apart.
Storage
Store at room temperature (below 30°C). Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
In case of ocular overdose, the eyes should be flushed with warm water. Systemic overdose is unlikely due to the low systemic absorption of ophthalmic drops.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administering to a nursing mother. Consult a doctor.
