Uses & Indications
Dosage
Adults
Typically 500 mg to 1 g intravenously every 8 hours, depending on the severity and type of infection. Infusion over 15-30 minutes.
Elderly
No specific dose adjustment is needed based on age alone, but renal function should be assessed and dose adjusted if renal impairment is present.
Renal_impairment
Dose reduction is required for patients with creatinine clearance less than 50 mL/min. For CrCl 26-50 mL/min: 500 mg every 12 hours; For CrCl 10-25 mL/min: 250 mg every 12 hours.
How to Take
Administer intravenously as an infusion over 15-30 minutes, or as an intravenous bolus injection over 3-5 minutes (for doses up to 1 g).

Contraindications

Drug Interactions
Probenecid
Concomitant administration of probenecid with meropenem inhibits the renal excretion of meropenem, resulting in increased plasma concentrations and prolonged half-life of meropenem. Co-administration is not recommended.
Valproic Acid
Reduced serum valproic acid levels, potentially leading to sub-therapeutic levels and loss of seizure control. Monitor valproic acid levels closely; consider alternative anticonvulsant therapy.
Oral Anticoagulants (e.g., Warfarin)
Increased anticoagulant effect, requiring close monitoring of INR.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze. Keep out of reach of children.
Overdose
In case of overdose, discontinue Meropenem and initiate supportive treatment. Meropenem can be removed by hemodialysis.
