Overview of the medicine
Ofex 200 mg Capsule contains Ofloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Usually 200 mg every 12 hours (twice daily) for 7-14 days, depending on the infection. For complicated UTIs, up to 400 mg twice daily. For uncomplicated gonorrhea, a single 400 mg dose.
Elderly
No specific dosage adjustment is required based solely on age, but renal function should be assessed and dose adjusted if impairment is present.
Renal_impairment
Dose adjustment is necessary. For CrCl 20-50 mL/min, 200 mg every 24 hours. For CrCl < 20 mL/min (including hemodialysis/CAPD), 100 mg every 24 hours.
How to Take
Take the capsule orally with a glass of water, with or without food. Do not chew or crush the capsule. It is important to complete the full course of treatment as prescribed by your doctor.
Mechanism of Action
Ofloxacin acts by inhibiting bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Side Effects
Contraindications
- Hypersensitivity to Ofloxacin or other fluoroquinolones
- History of tendon disorders related to quinolone administration
- Epilepsy or other seizure disorders
- Children and adolescents (due to risk of cartilage damage, generally avoided)
Drug Interactions
Warfarin
May enhance anticoagulant effects, requiring INR monitoring.
Theophylline
Increased plasma levels of Theophylline, potentially leading to toxicity.
Corticosteroids
Increased risk of tendon rupture.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Increased risk of CNS stimulation and convulsive seizures.
Antacids (containing aluminum, magnesium), sucralfate, iron, zinc supplements
Reduce oral absorption of Ofloxacin. Administer Ofloxacin 2 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include nausea, vomiting, abdominal pain, dizziness, confusion, seizures, and increased QT interval. Management includes emptying the stomach by induced vomiting or gastric lavage, followed by supportive care and symptomatic treatment. Adequate hydration should be maintained. Hemodialysis or peritoneal dialysis may not be effective in removing Ofloxacin.
Pregnancy & Lactation
Pregnancy Category C. Ofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
