Description
Overview of the medicine
Ofpain 60 mg Injection contains Ketorolac Tromethamine, a potent NSAID used for the short-term management of moderately severe acute pain requiring opioid-level analgesia.
Uses & Indications
Dosage
Adults
Initial dose 30 mg or 60 mg IM/IV once, then 15 mg or 30 mg IM/IV every 6 hours as needed. Maximum daily dose should not exceed 120 mg (for multiple-dose use) or 60 mg (for single-dose use) for patients under 65 kg.
Elderly
Lower initial dose (e.g., 15 mg every 6 hours). Maximum daily dose should not exceed 60 mg for patients aged 65 years and older, or those weighing less than 50 kg.
Renal_impairment
Reduced dosage required. Contraindicated in severe renal impairment. For moderate impairment, consider 15 mg every 6 hours, not exceeding 60 mg/day.
How to Take
Administered intramuscularly (IM) or intravenously (IV). For IM, inject slowly and deeply into a large muscle. For IV, administer over at least 15 seconds. Do not use for longer than 5 days.
Mechanism of Action
Ketorolac inhibits prostaglandin synthesis by blocking cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing pain and inflammation.
Pharmacokinetics
Onset
Onset of analgesia within 30-60 minutes after IM administration.
Excretion
Mainly renal (approximately 91%), with a small amount excreted in feces.
Half life
Approximately 4-6 hours.
Absorption
Rapid and complete absorption after intramuscular (IM) injection.
Metabolism
Primarily hepatic, via glucuronidation and hydroxylation.
Side Effects
Contraindications
- Hypersensitivity to Ketorolac, aspirin, or other NSAIDs.
- Active peptic ulcer, gastrointestinal bleeding, or perforation.
- Severe renal impairment or risk of renal failure.
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
- Coagulation disorders or patients at high risk of bleeding.
- Peri-operative pain in coronary artery bypass graft (CABG) surgery.
- Advanced liver disease.
- Concomitant use with other NSAIDs, aspirin, anticoagulants, or probenecid.
Drug Interactions
Lithium
Increased lithium plasma levels and toxicity. Monitor lithium levels.
Diuretics
Reduced diuretic and antihypertensive effects. Increased risk of renal impairment.
Probenecid
Increases Ketorolac plasma levels and half-life. Concomitant use is contraindicated.
Methotrexate
Increased methotrexate toxicity. Avoid concomitant use.
ACE Inhibitors/ARBs
Reduced antihypertensive effect; increased risk of renal impairment, especially in elderly or volume-depleted patients.
Other NSAIDs/Aspirin
Increased risk of gastrointestinal adverse events. Concomitant use is contraindicated.
Anticoagulants (e.g., Warfarin)
Increased risk of severe bleeding. Concomitant use is contraindicated.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms may include gastrointestinal irritation, nausea, vomiting, epigastric pain, lethargy, and drowsiness. Rare cases of severe effects like renal failure or respiratory depression. Management is symptomatic and supportive; there is no specific antidote. Gastric lavage or activated charcoal may be considered if recent oral ingestion, though for injection, supportive care is key.
Pregnancy & Lactation
Not recommended during late pregnancy (third trimester) due to potential adverse effects on fetal cardiovascular system (e.g., premature closure of ductus arteriosus). Use with caution during early pregnancy and lactation only if clearly needed and benefits outweigh risks. Consult a doctor.

YesClinical Trials
Extensive clinical trials have demonstrated Ketorolac's efficacy in managing acute pain, often showing comparable or superior analgesia to some opioids for short-term use, with a favorable safety profile when used as directed and for indicated durations.
Lab Monitoring
- Renal function (creatinine, BUN) should be monitored, especially in patients with pre-existing renal impairment or those on diuretics.
- Liver function tests (ALT, AST) periodically.
- Complete Blood Count (CBC) and coagulation parameters (e.g., prothrombin time, bleeding time) for prolonged use or if bleeding risk is present.
Doctor Notes
- Strictly for short-term (max 5 days) management of moderately severe acute pain where opioid-level analgesia is required and alternative non-opioid options are insufficient.
- Careful patient selection is crucial, especially regarding GI, cardiovascular, renal, and bleeding risks. Assess baseline renal function.
- Monitor for signs of bleeding, renal dysfunction, and GI adverse events throughout treatment. Discontinue if any serious adverse events occur.
- Avoid concomitant use with other NSAIDs, aspirin, anticoagulants, or probenecid.
Patient Guidelines
- This medication is for short-term pain relief only, typically not exceeding 5 days.
- Report any signs of stomach pain, black stools, unusual bruising, or bleeding immediately.
- Avoid alcohol while taking this medication as it may increase the risk of stomach problems.
- Do not exceed the prescribed dose or duration without consulting your doctor.
- Inform your doctor about all other medications you are taking, especially blood thinners or other pain relievers.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose.
Driving Precautions
Ofpain Injection may cause dizziness, drowsiness, or visual disturbances. Patients should be cautioned against driving or operating machinery until they are reasonably certain that Ketorolac does not adversely affect their ability to engage in such activities.
Lifestyle Advice
- Maintain adequate hydration to support kidney function.
- Adopt a balanced diet and avoid excessive consumption of fatty or spicy foods.
- Engage in light physical activity as tolerated to aid recovery and general well-being.
