Olanzapine is an atypical antipsychotic medication primarily used to treat schizophrenia and bipolar disorder. It helps to restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 5-10 mg once daily, target 10 mg/day. Bipolar mania: Initially 10-15 mg once daily.
Elderly
Lower starting dose (e.g., 5 mg/day) may be considered, with cautious titration.
Renal_impairment
No routine dosage adjustment is required, but caution is advised in severe cases.

How to Take
Take orally once daily with or without food. Oral disintegrating tablets should be placed on the tongue, where it rapidly disintegrates and can be swallowed with or without water.
Mechanism of Action
Olanzapine acts as an antagonist at multiple receptor sites, including dopamine D1, D2, D3, D4, and D5 receptors, and serotonin 5-HT2A, 5-HT2C, 5-HT3, and 5-HT6 receptors. It also has antagonistic effects on muscarinic M1-M5, alpha-1 adrenergic, and histamine H1 receptors.

Approximately 57% of a dose is excreted in the urine and 30% in the feces.

Contraindications
- Hypersensitivity to olanzapine or any component of the formulation
- Known narrow-angle glaucoma
Drug Interactions
Fluvoxamine
May increase olanzapine levels (CYP1A2 inhibitor); consider reducing olanzapine dose.
Carbamazepine
May decrease olanzapine levels (CYP1A2 inducer); consider increasing olanzapine dose.
Antihypertensives
May potentiate orthostatic hypotension.
Alcohol and other CNS depressants
May potentiate sedative effects and orthostatic hypotension.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include sedation, tachycardia, hypotension, extrapyramidal symptoms, and respiratory depression. Management is supportive, including maintaining airway, oxygenation, ventilation, and cardiovascular monitoring. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Not recommended during breastfeeding as it is excreted in breast milk.
