Description
Overview of the medicine
Olestar 4 gm Suspension contains Ursodeoxycholic Acid, a naturally occurring bile acid. It is used to dissolve cholesterol gallstones in patients who cannot undergo surgery, and to treat primary biliary cholangitis (PBC). It works by reducing the cholesterol content of bile, thereby preventing gallstone formation and promoting their dissolution.
Uses & Indications
Dosage
Adults
For gallstone dissolution: 8-10 mg/kg/day, usually given in 2-3 divided doses. For PBC: 13-15 mg/kg/day, usually given in 2-4 divided doses.
Elderly
No specific dose adjustment required, but monitor for adverse effects.
Renal_impairment
No specific dose adjustment required, as renal excretion is minimal.
How to Take
Administer orally with food, usually at bedtime or in divided doses. Shake the suspension well before use. Complete the full course of treatment as prescribed.
Mechanism of Action
Ursodeoxycholic Acid (UDCA) works by decreasing the cholesterol saturation of bile, inhibiting cholesterol absorption in the intestine, and suppressing cholesterol synthesis in the liver. It also increases bile flow and replaces toxic bile acids with less toxic ones, protecting liver cells.
Pharmacokinetics
Onset
Gallstone dissolution may take several months (6-24 months), symptomatic relief in PBC within weeks.
Excretion
Excreted primarily in feces, a small amount in urine.
Half life
Approximately 3.5 to 5.8 days.
Absorption
Well absorbed from the gastrointestinal tract, approximately 90%.
Metabolism
Undergoes extensive enterohepatic recirculation. Metabolized in the liver to active and inactive compounds, primarily by conjugation with glycine and taurine.
Side Effects
Contraindications
- •Acute cholecystitis
- •Cholangitis
- •Biliary obstruction
- •Calcified cholesterol gallstones
- •Non-functioning gallbladder
- •Pancreatitis
Drug Interactions
Cyclosporine
UDCA may increase cyclosporine absorption; monitor cyclosporine levels.
Estrogens, oral contraceptives, clofibrate
May increase cholesterol secretion into bile, counteracting UDCA's effects.
Cholestyramine, colestipol, or aluminum-containing antacids
May reduce UDCA absorption; administer UDCA at least 2 hours before or 4 hours after these agents.
Storage
Store below 30°C in a cool, dry place, away from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose are typically diarrhea. Treatment is symptomatic and supportive. No specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. UDCA is excreted into breast milk in very small amounts; generally considered safe during lactation, but consult a doctor.
